Health Promotion Program Needs Assessment
Evaluation of teaching provides the teacher with valuable data for quality assurance and improvement. For this assignment, you will develop two surveys. The first survey is the Teacher Evaluation Tool; it is intended to evaluate effectiveness of your presentation style. A second survey is the Learning Objectives Evaluation Tool. This tool will evaluate whether your three behavioral learning objectives were met. It is expected that you will print and distribute both evaluation tools for your audience to complete. You will summarize the aggregate data of these surveys in your final reflection paper. This week, upload both surveys as Word documents (.doc or.docx) by the day/time listed. Please review the Grading Rubric and the sample evaluations provided in the course.
Tool #1 – Teacher Evaluation Tool
The Teacher Evaluation tool allows the audience to evaluate your teaching effectiveness. After presenting, distribute the Teacher Evaluation tool to your audience and aggregate the results for use in your final reflection paper.
Create a tool that is easy to use. Start with clear directions. Explain that the results are anonymous and will be used to help you grow as an educator. Write easy-to-read questions that allow the audience to circle or checkmark their responses. The evaluation should require 2 minutes or less to complete. Consider health literacy in your sentence structure. Below are some sample questions:

Tool #2 – Learning Objectives Evaluation Tool
The Learning Objectives Evaluation tool evaluates whether your health education resulted in the learner meeting your stated behavioral learning objectives. After teaching your audience, distribute the Learning Objectives Evaluation tool and aggregate the results for use in your final reflection paper.
