Using the Prospectus Form, complete Sections 1-5 according to the appendix instructions, examples, and resources.

Students will also be required to submit their 5 PDF supporting sources for the below sections of the form.

· Project Planning (Prospectus) form

· 1 PDF supporting the Problem of Interest.

· 3 PDF’s supporting the Healthcare Administration Problem.

· 1 PDF supporting the ideal state of operations.

Include the PDF for each sources. Reviews the prospectus Form and add more details if needed.
