Unfortunately there are a number of shortcoming in this submission that will require you to submit again.
For the first task you are asked to justify whether his glycaemic control should be increased and the reason for this. You have written about the WHO diagnostic criteria but these are not relevant as the patient has already been diagnosed with diabetes. You then go onto to state : "ADA recommendation is supported by the evidence that the low HbA1c targets never minimise the risk of the comprehensive macrovascular complications VADT, ACCORD, and ADVANCE".
This seems to suggest that you think that lowering the HbA1c target is detrimental to macrovascular risk. This is at best an inaccurate interpretation of the ADA guidelines. Moreover if you follow this thought through it means that you are considering not giving this patient any further diabetes treatment (in addition to metformin). If this is not what you mean you need to be more careful about how you are presenting your ideas. For the remainder of this first task you need to select a HbA1c target and provide the reasons for this using published evidence and guidelines.
For task 2 you have managed to give a list of possible medications for this patient and then discussed the mode of action, benefits and risks. Bromocriptine is an unusual choice for this patient - how often do your patients get this prescribed after metformin? Do you have any evidence for what is happening in your local health economy with respect to bromocriptine prescribing?
For task 3 you have given the mode of action of all the drugs and then discussed the benefits of all the drug and then all of their risks. I would suggest a better way of doing this is to write about each drug class at a time. This helps to build a picture of which drug is likely to be helpful for this patient.
For task 5 you are asked to provide a detailed case for the drug you have finally recommended for this patient. You have written very little and so you have not been able to present a convincing case for the prescribing of DPP4 inhibitors for this patient.
Finally your choice of references seems very limited and does not cover the major guidelines or trials that are relevant .
