3A) Background
The population health assessment revealed significant limitations to the surveillance used by the healthcare system in City C. Limited data was available on pertinent mental health outcomes, and only
recently could they examine any outcomes by gender and sexual identity.
The stakeholders advisory board recommended improving the population
health surveillance system being used by the healthcare system in City C, so that they could examine the extent to which the healthcare system made improvements in their ability to treat mental health issues, particularly for their patients who identify as LGBTQI. In addition to relying on the expertise of the stakeholder’s advisory board, national recommendations for mental health surveillance such as the guidance provided by the CDC, were used to improve the healthcare system’s surveillance system (Azofeifa et al., 2018).

Imagine that you have been asked by the healthcare system’s Task Force to improve upon the existing population health surveillance system. What are the best practices for population health surveillance that you would ensure are embedded in the healthcare system’s surveillance system?
3C) Considerations
Consider the following ways in which City C’s healthcare system could incorporate best practices for population health surveillance to improve their monitoring and tracking of mental health outcomes, especially in those who identify as LGBTQI.
Timely
· The surveillance system should incorporate regular tracking of pertinent mental health data that can be routinely reviewed to ensure that as the healthcare system makes changes, those changes are improving mental health outcomes. The stakeholders advisory board recommended monthly monitoring of mental health outcomes.
Consistent
· Systems and protocols will be created to ensure that the mental health outcomes are consistently measured to ensure data integrity. For example, the surveillance system will use electronic health record data (EHR) to track outcomes such as adult binge drinking prevalence and prevalence of past year any mental illness. The outcome definitions will be standardized by using ICD-10 codes and/or valid and reliable survey questions. Any changes in the way these outcomes are defined/measured will be reviewed and documented by a data management team.
· The systems and protocols will be implemented across all of the healthcare system’s hospitals and clinics, with routine checks by the data management team to ensure consistency.
Broad
· In addition to examining gender and sexual identity via questions recommended by experts, the health system will expand upon the data they collect to include comprehensive information on social determinants of health to understand how social determinants impact their patients’ mental health.
· Relevant social determinants of health include food and housing insecurity, employment, and transportation access.
Representative
· Given the stakeholder advisory board’s concern about improving mental health care for patients who identify as LGBTQI, and the historical inability to examine mental health outcomes in LGBTQI patients by the healthcare system, surveillance efforts prioritized inclusion of those who identify as LGBTQI.
· As noted above, recommended questions for identifying LGBTQI individuals in healthcare settings were adopted.
· Surveillance efforts also prioritized inclusion of intersecting minoritized identities (e.g., patients who identify as gay and Black), since they may be at increased risk for poorer mental health.
Variety
· Use of EHR data is a key aspect of the healthcare systems surveillance plan, though other sources of data were incorporated.
· Geographical information systems (GIS) data were connected to EHR to determine if specific populations may have less access to mental health services due to transportation and/or clinic location barriers
· Social media data about the healthcare system were tracked to understand how community members perceive the healthcare system and their ability to provide mental health care.
