A 38-years old Mary visited the outpatient clinic for his routine scheduled regarding her conception. The nurse realized that her blood pressure has increased based on her previous records, she drinks about 10-12units of alcohol but does not smoke. On examination her blood pressure was 158/94mmHg while her heart rate is 72 beats per minutes and regular. She not been diagnosed of hypertension before, so she does not use any antihypertensive medications.

Medical Disease and Differential Diagnosis.

The nurse suspects hypertension. When considering a high blood pressure diagnosis, the nurse should measure both arm’s blood pressure in three consecutive readings. If the hospital’s blood pressure is 140/90mmHg or more, the nurse should document the lower between the final two reading as the hospital blood pressure (Melillo et al.,2015). The nurse with the responsibility of assembling the assessment equipment should ensure that it takes at least two readings per hour during the regular working hours. The average measurement of al least 14 readings taken during the working confirms high bleed pressure diagnosis. Mar’s average readings was 164/92mmHg 9 (Melillo et al., 2015). At the same moment, the nurse should consider doing investigations that target damage to organs like hypertension, retinopathy, and kidney disease.

Hypertension is a common cardiovascular disorder that if not well-managed, can cause severe health complications. These include stroke, cardiac failure, and kidney injury. According to the health assessment, there are various issues that Mary should consider changing in her lifestyle. As a result, the planned patient education content should comprise information about her current blood pressure compared to the normal blood pressure levels, self-assessment techniques and their importance (week 2 lesson, 2020). Besides education will include encouraging regular exercise programs like walking, jogging between 90-150 minutes weekly. Another area of concern in patient education is nutritional changes. Example Mary will reduce the sodium consumption which will help maintain the blood pressure at a borderline. Also, Mary will be provided with learning publication like pamphlets and periodicals that discusses hypertension and diet regimen. Finally, the nurse will determine the daily dose of hydrochlorothiazide 25mg to use along diary changes and physical exercise (Himmelfarb et al. 2016)

Blood Pressure Log-MyDiary.

The most appropriate mHealth app for Mary is MyDiary. It is an app the monitors personal journals and records their movements, including their dietary data, physical exercise, medications adherence, daily readings. Clinical features and adverse effects of drugs. Other features found on the app are images, blood pressure, temperature, pulse rate and weight. Mary can use her Android phone or tablet to access the MyDiary app to monitor her blood pressure on daily bases, truck medication, when to exercise, what diet or food is appropriate to manage the high blood pressure. Thus, it is developed to help any person assess and manage their high blood pressure.

The app was developed for everyone, and the information in it is generally suitable for every age group. To facilitate improved access to patients' treatment, including follow up every time, convincing several people to use it. A patient can record as many symptoms are needed, set when they feel an improvement, and eventually post their clinical features journal when they feel like doing so. Thus, it is no longer necessary to record and print journal templates in either pdf or word documents.
