In this assignment you will practice identifying and analyzing various failure modes within a process. Failure Mode and Effects Analysis (FMEA) is a risk-assessment technique for evaluating the severity, occurrence, and detection of risks or uncertainty within a process. FMEA identifies possible failures and its effects on the process. Assigning failure modes numeric values or risk priority numbers (RPN) that quantify the possibility of the failure occurring, being detected, and its severity is facilitated by an FMEA effects analysis. To complete this assignment, first review the Case Study: Applying Statistical Analysis in a Six Sigma Project at GE Fanue (pp. 186-190). Then, address the following in about a paragraph each for your submission:
● Identify two possible or noted failures and analyze the effects of each failure ● Calculate the RPN (severity, occurrence, detection) for each identified failure and justify your assessment. Refer to How To Perform a
Failure Mode and Effects Analysis Tutorial for assistance.
