Scenario is clear and concise, including a disease process, diagnosis, OR identify a patient with a desire to maintain good health and prevent illness. Include the nurse’s assessment of learning needs and readiness to learn.

Mr. NB, a 49 years old moderately obese man, came to the emergency department with a month's history of progressive headache presented to the emergency department with nausea, vomiting, and visual disturbance. Headache was unrelieved by medication. It is noted that he was previously diagnosed with hypertension (HTN) and was prescribed with Beta-blockers but has not taken medicine for a year now for an unspecified reason. He is an active tobacco smoker and drinks alcohol occasionally. Denies of recreational drug use. He has family history of HTN to both father and mother. On arrival, his blood pressure was noted to be 180/110. MRI of the brain demonstrated scattered lesions with increased intensity in the occipital and posterior parietal lobes. But no acute CVA noted. The clinical presentation is suggestive of hypertensive emergency and hypertensive encephalopathy. He has a college-level education. After knowing about his current condition and future possibility of having stroke, the patient is more willing to learn about the ways to control his blood pressure and bring change in lifestyle.

As per the information obtained from Mr. NB's assessment, it was revealed that he needs to modify his lifestyle, and proper education is necessary to the patient. Education will be more focused on lifestyle modification that includes encouraging exercise and reducing weight, dietary changes, and consultations with a dietician/nutritionist. It will be beneficial if the patient is taught about self-monitoring skills, provide information on their target blood pressure, and tell them about its importance to maintaining the targeted blood pressure. We will provide education on the medication regimen he will be in as the patient had previously stopped taking medication. He will be provided with educational materials like pamphlets about the disease condition, sample diet like low salt diet and cardiac diet, and encouragement to light exercise with walking. Mr. NB will also be discussed in detail about the medication regimens that include when and how to take medicine, how the drug works, and the adverse effect. The addition of hydrochlorothiazide 25mg daily three times a day and metoprolol 50mg twice a day.

A mHealth app that could benefit Mr. NB would be the CareClinic App. It is a mobile app available on Android, iPhone, and iPad. This app can be downloaded from Apple Appstore and google play store for free of cost. You can also login through your computer to the CareClinic website. The app can be used to track symptoms, activity, dietary intake, and medication reminders (Careclinic.io, 2020). It is intended to assist anyone with management and monitoring for hypertension. It helps to help personalize your health monitoring and make it easier to see where you can adjust strategies to enhance your hypertension management. The app is rated for everyone it has been deemed that the content is generally suitable for all ages. This app will allow easy monitoring and management of hypertension, making people more apt to utilize. The app not only helps in tracking blood pressure but also has other features that includes a reminder to engage in physical activity, eat correctly, remind you to take medicines and much more through care plans.
