Overview
In this module, you learned how tools and technology can help with the documenta!on process. They make regulatory
compliance and audits easier for healthcare organiza!ons.
This ac!vity will help you assess how technology supports regulatory compliance in healthcare. It also enables you to explain
how electronic documenta!on systems support billing and payment reimbursement.
Scenario
You are the manager of the coding department for a healthcare organiza!on and have found a coding error while doing a
preliminary compliance audit of claims submission to Medicare. You found that the physician has entered incorrect procedure
codes on encounter forms and found similar errors on other suppor!ng documenta!on, such as pa!ent visit summaries,
electronic medical records, etc., for the last quarter. You also no!ce that the documenta!on and coding policies and
procedures have not been updated in three years. You need to examine the situa!on and recommend ac!ons to mi!gate the
error.
Direc!ons
Create a situa!on, background, assessment, recommenda!on (SBAR) analysis using the template provided to assess the
documenta!on and coding scenario and recommend ways to rec!fy the situa!on. Explain the situa!on in detail, followed by
background informa!on. Include any assump!ons you are making about the situa!on. Then explain the immediate
assessment and recommenda!ons you have for your supervisor.
You may want to review the resources in this module to refresh your knowledge of how to create an SBAR analysis. If you
need wri!ng support, access the Online Wri!ng Center through the Academic Support module of your course.
Specifically, you must address the following rubric criteria:
1. Situa!on: Analyze the situa!on for elements of concern rela!ng to compliance and financial reimbursement. Consider
the following ques!ons to guide your response:
What is the problem, and how or where did it begin?
How severe is the problem as it relates to organiza!onal risk and financial performance?
2. Background: Explain the essen!al informa!on related to the compliance and reimbursement situa!on. Consider the
following ques!ons to guide your response:
What is the current status of the situa!on?
Who should receive communica!on about the problem?
3. Assessment: Assess the concerns adequately based on the situa!on and the background informa!on. Consider the
following ques!ons to guide your response:
What is your understanding of the situa!on, and why must it be addressed immediately?
What are your assump!ons about the root cause of the problem?
4. Recommenda!on: Recommend at least two ways to resolve the problem based on the situa!on, background, and
your assessment. Consider the following ques!ons to guide your response:

How would you educate the physician and other providers using evidence-based research and guidelines?
How would you deliver your recommenda!on in various learning environments?
All the claims in your deliverable should be evidence based. Your cita!ons should be from your independent search for
evidence (not from the textbook or module resources) of credible sources and be current within the last five years. You are
required to cite a minimum of two sources. Refer to the IHP 630 Library Guide located in the Start Here sec!on of the course
for addi!onal support.
What to Submit
Submit your completed SBAR Analysis template as a 1- to 2-page Microso" Word document with double spacing, 12-point
Times New Roman font, and one-inch margins. Sources should be cited according to APA style.
