This assignment contains 8 questions for the total of 70 points.

Data : HBMcovid 19.sav is attached to a dropbox folder in folio.

This data contains 864 entries from self-reported measures on community adults reporting about

COVID-19 vaccination intention, health belief model components (perceived threat, benefits of

vaccination, barriers to vaccination, self-efficacy) and pandemic worry . Data was collected during

the first state of emergency declared in March 2020 in Romania.

Main Research Question:

Are there components of the health belief model (perceived threat, benefits of vaccination, barriers

to vaccination, self-efficacy) associated with vaccination intention controlling for gender and age?

All components of HBM were measured on a 6 point Likert scale.

1 = Strongly Disagree

2= Disagree

3= Slightly Disagree

4= Slightly Agree

5 = Agree

6= Strongly Agree

Each participant in this survey was given ONE question related to the intention of getting the

vaccine. This question was measured on a binary scale (Intentionbinary). 1=Participant will take

the vaccine, and 0=Participant will not take the vaccine. The name of this binary variable in your

SPSS dataset attached in the dropbox is Intentionbinary.

1. (5 points) Each survey participant was provided with four questions related to the perceived

threat of COVID-19. These four questions have been measured on a scale of 1-6. 1= Strongly

disagree, and 6 = Strongly agree. Create a total perceived threat score for each participant

using the SUM of these four questions (threat 1-threat 4). For anyone participating in this

study, the maximum score may be 24, and the lowest score may be 4.

2. (5 points) Each survey participant was provided with five questions related to the perceived

benefits of COVID-19 vaccination. These five questions have been measured on a scale of 1-6.

1= Strongly disagree, and 6 = Strongly agree. Create a total perceived benefit score for each

participant using the SUM of these five questions (benef1-benef5). For anyone participating in

this study, the maximum score may be 30, and the lowest score may be 5.

3. (5 points) Each survey participant was provided with ten questions related to the perceived

barriers of COVID-19 vaccination. These ten questions have been measured on a scale of 1-6.

1= Strongly disagree, and 6 = Strongly agree. Create a total perceived barrier score for each

participant using the SUM of these ten questions (barier1-barier10). For anyone participating

in this study, the maximum score may be 60, and the lowest score may be 10.

4. (5 points) Each survey participant was provided with five questions related to the self-efficacy.

These five questions have been measured on a scale of 1-6. 1= Strongly disagree, and 6 =

Strongly agree. Create a total self-efficacy score for each participant using the SUM of these

five questions (efic1-efic5). For anyone participating in this study, the maximum score may be

30, and the lowest score may be 5.

5. (10 points) Are all or some of the logistic regression assumptions for the main research question

of interest satisfied? Show your work in support of your conclusion.

6. (10 points) Perform the logistic regression analysis to address the main research question of

interest. (Hint: Intentionbinary is your dependent variable. Age , gender and total score

variables for each components are your independent variables. In this dataset age is a categorical

variable (see values for age in the dataset). Perform the analysis regardless of whether the

assumptions met in the previous question. )

7. (15 points) Report and interpret your results for main research question of interest in terms of

β coefficients.

8. (15 points) Report and interpret your results for the main research questions of interest in

terms of AOR (adjusted odds ratio).
