Problem Statement:
Older adults are at risk have several complex medical and social needs that are fatal and can result in high increased mortality rates among older adults if care management for these adults is not efficient and effective. However, due to poor care management practices in hospitals, there has been an increase in the rate of rehospitalization rates which has been alarming. One of the strategies that can improve care management is pay-for-performance incentives to help reduce the mortality rates of older adults and reduce rehospitalization rates among these individuals.
