The EHR has become more accessible to most hospitals now than the past 20 years. All health care workers can now provide a more comprehensive process of daily care by seeing past visits at other entities who have given care to the same patient. This also is a good way to review medications given and prescribed at discharge, tests ordered and performed, care plans created, or discharge locations that are not done to the patient's home.

As software is developed over time there have been internal processes made easier for reporting, tracking and producing percentages of population areas who have limited access to healthcare, provides reports on provider percentages for performed surgery, patients now can have access to their own record based on tests ordered and performed, patients have immediate access to these tests results, they now have the ability to pay their bill on-line, schedule doctors office appointments, request medication refills on-line, and also have the ease of use for telehealth prompted visits.

Tracking for a potential health care risk is only as good as the person documenting the information but the database needs to be built that provides the assessor to enter the information to a particular field that would make a tool easiest to find when running the report.
