Assignment Instructions: The purpose of this assignment is for you to develop your Personal Emergency Preparedness Plan. The following steps will guide you in developing and documenting your plan. Steps 1 -4 are items for you to consider addressing in your plan. Steps 4-7 are the minimum requirements for this assignment / your plan. 1) Know Your Risks:

• What risks are possible in the area where you live? Resource: US Natural Hazards Index

• What risks are possible in the area where your family lives? Resource: US Natural Hazards Index • What measures should be taken to prepare for identified risks? Resources: Disasters and Emergencies (DHS)

2) Identify the needed items in your Emergency Supply Kit: • Put together a list of basic disaster supplies that you will need during an emergency • Consider locations to store your kit for different situations

• Identify someone in your community or family that you can assist in doing the same

3) Gather information necessary to create your plan by addressing the following:

• How will you receive emergency alerts and warnings? (Resources) • What is your shelter plan? (Resources)

• What is your evacuation route? Be sure to address your evacuation plan before, duri ng, and after your evacuation. (Resources)

• What is your communication plan? (Resources) What happens when technology is not available? (Resources)

• Financial Preparedness (Resource)  What critical documents and valuables do you need to preserve?  Have you documented your valuable property to submit for loss?  Have you secured appropriate insurance for your valuables?  Have you considered your emergency finances?

4) Identify the specific needs in your household and discuss the specific areas where you will need assistance.

Consider the following factors when developing your plan: • Different ages of members within your household • Responsibilities for assisting others (Resource)

• Locations frequented (Resource)

• Dietary needs • Medical needs including prescription medications and equipment

• Disabilities or access and functional needs including devices and equipment (Resource) • Languages spoken

• Cultural and religious considerations

• Readiness for College Students (Resource) • Pets or service animals – (Resource)

• Households with school-aged children (Resource 1; Resource 2 – Sesame Street)

5) Create your individual / family plan.

Resources to assist you with this step can be found at www.ready.gov/make-a-plan. Your plan should contain at least the following sections:

o Identification of Potential Hazards (10 points) o Family Considerations (50 points)

o Disabilities o Children o College o Religious o Pets

o Disaster Supplies Checklist (10 points) o Dietary Needs (10 points) o Medical Needs (10 points) o Emergency Alerts and Warnings (10 points) o Location information (10 points) o Shelter Plan (10 points) o Evacuation Route (10 points) o Communication Plan (10 points) o Documents / Valuables / Property Information / Insurance / Finances (10 points)

The requirement of APA resource lists and in-text citations is to ensure that you are able to update the plan at a later date. Additionally, you should stay up -to-date with the applicable resources and publications.

• Is there detailed inf ormation presented so that the plan is transf erable? (10 points)

TOTAL POINTS POSSIBLE: 300 The total points earned will be converted to a percentage score for inclusion in your overall course average. General Notes:

 The medium you use to design your plan is up to you. It can be a website, shared drive, or document with updatable links. Whatever method you use, it should be easily accessible to those who are included in t he plan (signif icant other, f amily, f riends, etc.)

 While you are encouraged to utilize the template resources, your plan MUST NOT be merely a compilation of templates obtained f rom the various resources provided in this document.

 This assignment will be submitted in Canvas and can be either a link to a set of resources or upload of a

document.
