Critical reflection of your growth and development during your practicum experience in a clinical setting has the benefit of helping you to identify opportunities for improvement in your clinical skills, while also recognizing your strengths and successes.

Use this Journal to reflect on your clinical strengths and opportunities for improvement, the progress you made, and what insights you will carry forward into your next practicum

TO PREPARE

· Refer to the “Advanced Nursing Practice Competencies and Guidelines” found in the Week 1 Learning Resources, and consider the quality measures or indicators advanced nursing practice nurses must possess in your specialty of interest.

· Refer to your “Clinical Skills Self-Assessment Form” you submitted in Week 1, and consider your strengths and opportunities for improvement.

· Refer to your Patient Log in Meditrek, and consider the patient activities you have experienced in your practicum experience. Reflect on your observations and experiences.

In 450–500 words, address the following:

Learning From Experiences

· Revisit the goals and objectives from your Practicum Experience Plan. Explain the degree to which you achieved each during the practicum experience.

· Reflect on the three (3) most challenging patients you encountered during the practicum experience. What was most challenging about each?

· What did you learn from this experience?

· What resources did you have available?

· What evidence-based practice did you use for the patients?

· What new skills are you learning?

· What would you do differently?

· How are you managing patient flow and volume?

Communicating and Feedback

· Reflect on how you might improve your skills and knowledge and how to communicate those efforts to your Preceptor.

· Answer the questions: How am I doing? What is missing?

· Reflect on the formal and informal feedback you received from your Preceptor.

here is my practicum experience plan:

Refer to the instructions in Week 2 to create individualized practicum learning objectives that meet the requirements for this course. These objectives should be aligned specifically to your Practicum experience. Your objectives should address your self-assessment of the skills found in the “PMHNP Clinical Skills Self-Assessment Form” you completed in Week 1.

As you develop your individualized practicum learning objective, be sure to write them using the SMART format. Use the resources found in Week 2 to guide your development. Once you review your resources, continue and complete the following. Note: Please make sure each of your objectives are connected to your self-assessment. Also, consider that you will need to demonstrate how you are advancing your knowledge in the clinical specialty.

Objective 1: By the end of the practicum period, I will be able to differentiate between pathophysiological and psychopathological conditions when assessing and treating individuals across lifespan.

Planned Activities: By the end of this practicum period, I will be able to apply knowledge of the difference between pathophysiological conditions and psychopathological conditions on my clients, I will use comprehensive assessment skills based on evidence-based practice. I will be able to identify differential diagnoses and develop an appropriate working diagnosis. I will work closely with my preceptor to achieve these goals.

Mode of Assessment: Over the next 160 clinical hours, I will be able to assess client, diagnose and suggest possible treatment plan to my preceptor for review. These will improve my knowledge of the differences between psychopathological and pathophysiological conditions (Note: Verification will be documented in Meditrek)

· Develop comprehensive psychiatric evaluation skills in differentiating between psychological conditions and pathological conditions

· Apply advance nursing assessment and diagnosis skills in mental health

Objective 2: By the end of the practicum period, I will be able to provide psychoeducation to the patients and their caregivers to facilitate treatment

Planned Activities: By the end of this practicum period, I plan to provide psychoeducation to my patients and their caregivers, I plan on conducting further research on the psychoeducation process. I also plan on conducting more research on the benefits of psychoeducation for the patients and their caregivers. In addition, I plan on seeking assistance from my preceptor and other senior colleague on how to provide psychoeducation effectively. I plan on providing psychoeducation to at least 10 patients and their caregivers.

Mode of Assessment: Over the next 160 clinical hours, I will perform psychoeducation to both patients and their caregiver under the supervision of my preceptor. This will help me to be more confidence and improve my psychoeducation skills.

Objective 3: By the end of the practicum period, I will be confidence in utilizing and interpreting screening tools for psychopathology.

Planned Activities: By the end of this practicum period, I will be able to select the appropriate screening tool, implement it and interpret the result appropriately for any clinical condition under the supervision of my preceptor.

Mode of Assessment: Over the next 160 clinical hours, I will identify and utilized different screening tools for appropriate clinical conditions.

Part 3: Projected Timeline/Schedule

Estimate how many hours you expect to work on your Practicum each week. *Note: All of your hours and activities must be supervised by your Preceptor and completed onsite. Your Preceptor will approve all hours, but your activities will be approved by both your Preceptor and Instructor. Any changes to this plan must be approved.

This timeline is intended as a planning tool; your actual schedule may differ from the projections you are making now.

I intend to complete the 144 or 160 Practicum hours (as applicable) according to the following timeline/schedule. I also understand that I must see at least 80 patients during my practicum experience. I understand that I may not complete my practicum hours sooner than 8 weeks. I understand I may not be in the practicum setting longer than 8 hours per day unless pre-approved by my faculty.
