Part 1: Dice Roll
Before completing the written portion of the journal assignment, you must complete the dice roll. During the dice roll, you will identify specific characteristics of your “person”.
Instructions:
Roll one die one time for each set of characteristics listed below. Your “person” will have the characteristics for the corresponding numbers listed.
Characteristics:
1. Has your person had a heart attack?
a. 1, 3, 5 = YES
b. 2, 4, 6 = NO (if NO, skip to Question 3)
2. How are they doing?
a. 1, 3 = They are doing well, no long-term problems seen
b. 2 = Doing ok but have occasional symptoms
c. 4, 6 = They are doing fair although they receive regular heart checks
d. 5 = They are doing poorly and are at high risk for a second heart attack
3. What other health conditions does your person have?
a. 1 = Arthritis
b. 2 = Alzheimer’s disease
c. 3 = Diabetes
d. 4 = Obesity
e. 5 = Cancer
f. 6 = None
4. If your person was married in Journal 5, is the spouse still living? Skip to question 7 if your person was not married in Journal 5.
a. 1, 3, 5 = YES (move on to question 5, DO NOT complete question 6)
b. 2, 4, 6 = NO (skip to question 6)
5. What is the spouse’s health?
a. 1, 3, 5 = Doing poorly and has at least one chronic illness
b. 2, 4, 6 = Doing well physically and emotionally
6. What stage of grief is the individual in?
a. 1 = Denial
b. 2 = Anger
c. 3, 4 = Acceptance
d. 5 = Depression
e. 6 = Bargaining
7. Does your person now have grandchildren? (If your person had no children, skip to question 9)
a. 1, 2, 3 = YES (move on to question 8)
b. 4, 5, 6 = NO (skip to question 9)
8. What is their relationship with them?
a. 1, 3 = Geographically distant but emotionally close
b. 2, 6 = Geographically and emotionally distant
c. 4, 5 = Geographically and emotionally close
9. How many close friends does your person have?
a. The number you roll is the number of friends your person has
