Created a plan for successful completion of the course with the Individual Success Plan. Did research on 4 topics

for capstone proposal that reflected on my community and everyday patient population that I care for. Met with my

preceptor this week to discuss class objectives, goals, and collaborate about the topic for my proposal. After

starting the conversation with my preceptor about the change proposal, we agreed that it should be something we

both see as an issue that we deal with daily.

My preceptor list me Common Medications That Can Cause Falls and Potentially Inappropriate Meds: Medications

to Avoid in Older Adults. We talk about fall prevention mnemonic 4 P, (Pain, Position, potty, Personal items). We

found with eight peer reviewed articles that help to support my topic for my project. The feedback from my preceptor

allowed me to develop in my nursing practice for Capstone Project.

This week Spoke with preceptor on topic for project and weekly objectives. Completed capstone summary and

Capstone Project Topic Selection and Approval. Identified the Risk for falls is a common nursing diagnosis among

elderly patients. Fall continues to be the leading issue of elder mortality, a detailed description with support, the

significance and implications for nursing practice, the proposed solution. We found with eight peer reviewed articles

that help to support my topic for my project.
