Agency Intervention/Treatment Plan
New Horizons Mental Health treatment plan identifies a problem narrative of a client to determine what types of social services are appropriate for every client. After determining what the presenting problem, the agency, and the client collaborate on measurable and achievable goals with a target date of achieving specific goals. The agency and the client then collaborate on objectives narrative to achieve the client’s goal and provide at least two interventions to assist in the objectives of the clients. So if a client has one main goal that may be composed of four different objectives to achieve that one goal, the agency social workers provide two intervention skills per objective in assisting with individual objectives. As part of the treatment plan is that all initial clients will be referred to the doctor and nurse with having a diagnostic assessment; a service plan development as part New Horizons policy and procedures to assist clients any other issues which may be necessary.
