Draft up a logic model to show how your intended “program” (service, intervention) works. Be sure to include:
a. A model (diagram) of the resources/inputs, activities, outputs, and outcomes (short and long-term) of the program. See examples. (points 7)
b. In a few sentences, briefly describe the program (don’t forget to include the name of the program-you can adapt existing programs that exist). You’ll expand upon the description of the program in your proposal. (points 2)
c. In a short paragraph, briefly describe where the program is being implemented or could be implemented (this can be hypothetical). If you have firsthand knowledge of the program, describe the setting where it is being implemented. If you are selecting a program that you are interested in and have no firsthand knowledge of the program (other than what you found in the literature), then identify a location (agency/organization, school, etc.) where the program could be implemented. Be realistic when developing the model (ask yourself, what would it take to get this program up and running so that you could evaluate the outcomes of it). (points 1)
d. Page length = 1 to 2 pages (please don’t use small font in the table, use at least 10 size font).
e. Include your name on all assignment or title page.
