Valley Hospital Stage 1: Inform Me

Welcome to Valley Hospital! A three-year strategic plan has recently been finalized for Valley Hospital, and a focus on quality, patient-centeredness and growing market share has emerged as major themes. The Chief Medical Officer (CMO) has been assigned to lead the patient-centered initiatives, including improving patient engagement. You are the Chief Medical Officer (CMO) of Valley Hospital. The first step that you take is to evaluate your current state with patient engagement. You have chosen to use the NeHC Framework as a guide through this project. Your initial focus is to determine whether your organization has meet any of the requirements for the “Inform Me” stage 1. Click on the button labeled background to learn more about the hospital’s situation, and then when you are ready to start gathering information to help you decide on what kind of initiatives you’d like to pursue, just enter the hospital by clicking on it.

Slide Eight:
Introduction: You now move on with your clipboard to a patient care unit in the hospital. You interview the team there about patient education materials available. You describe to the nurse about this initiative for the hospital of patient engagement and state: A paradigm shift has been occurring in healthcare from a paternalistic approach to a patient-centered approach where the patient is intimately involved with his or her plan of care. This shift from physician-centric to patient-centric care is here to stay. New demands are being placed on healthcare organizations to develop innovation approaches to engage their patients using technology. So as Valley Health prepares to deal with healthcare reform and other challenges, they are developing multiple strategies that include patient engagement on their agendas.
Female Nurse VO: That makes sense. We do a lot for patient engagement already. There are different types of patient education that our patients receive, a variety of care plans, medications, treatments, procedures, and tests. We bring in the computer on wheels into the patient’s room and discuss with the patient what goals we have set for him in his care plan while he is in the hospital. We also give educational sheets that we can leave with the patients regarding their medications. We have a file cabinet full of preprinted sheets that we hand out to patients regarding treatments and procedures. Sometimes we have sheets that are specific to individual physicians. However, for tests we haven’t given anything beyond a verbal explanation. So Doc, what kind of shape are we in for this patient engagement stuff?
Female Doctor VO: Based on what the CMO has shown me so far, we are not fully compliant with Stage 1 on patient education but are partially there. It seems like we are on track with care plans and medications, but I think we have some work to do with treatment, procedures and tests. We need to move these to an online format.
Female Doctor VO (if all slides 3-5 correct): We also need to consider a listing of physicians. More and more often hospitals and physicians are being graded by the public, and patients are comparing this information when making choices. We also need to have a comprehensive list of services at Valley Hospital. Our awards, while important, don’t really advances us to Stage 1.
Male Nurse VO: What about other types of paper that we will share with the patient and get signatures on, like informed consents and advance directives? Do you think it might be possible to make these forms available online and more interactive? I think what we are talking about here is Meaningful Use. I saw something in the hospital newsletter about steps the hospital has to take to be Meaningful Users of technology. The hospital can get incentive funds if we adopt early, and that way we can avoid financial penalties in the future. I think Meaningful Use requirements do overlap with some of the patient engagement initiatives, and that over the next several years there is going to be a lot of

new initiatives with participation from all areas of the hospital, including information technology, marketing, education, nursing, and administration. But, what about when the computers go down, and patients who don’t like computers, and doesn’t all this cost money that we don’t have?
