· Please carefully read the “Event and Causal Tree” doc. which gives explanation on this knowledge.

Instruction: Based on the information from the “Background “ and your research. Perform a Root Cause Analysis of the Uber’s Operational Failure by creating an Event and Causal Tree using the template named “Event and Causal Tree” doc.

The assignment based is divided into two parts:

1. Build a “Event and Causal Tree” for Uber’s Operational Failure, you can refer to Figure 3.4 in “Event and Causal Tree” doc.

2. Write a few paragraphs to illustrate the created tree. 400 words.
