Subject: Injury Incident of Mrs. Lopez in Sunset Estates Nursing Home

Type of incident

Mrs. Lopez a bedridden patient in our facility at the sunset estates nursing home sustained injuries on her arms and legs after falling from her Medi-Cal bed. The injuries occurred on the night of October 11, 2017 while she was sleeping. Following her injuries, Mrs. Lopez was attended by a doctor where after her injuries were stitched and her fingers fixed she was prescribed some medication to take.

Description of the incident

On the night of October 11, 2017 Elena Thompson, a nurse’s aide at our nursing home found that Mrs. Lopez, a bedridden patient, had fallen out of bed. Mrs. Lopez had bruises on her legs and her three fingers were bent at an awkward angle, suggesting they may had fractured. Her arms and legs were cold, indicated that she had been out of bed for some time. At the time she was found she was awake but she had no idea on how she had come out of bed.

After the incident

After learning of the incident I quickly acted by immediately calling a doctor to attend to Mrs. Lopez. After ensuring that Mrs. Lopez was well taken care of I begun conducting my investigation to see why the incident had happened. I started by talking to the janitorial staff and the aides who were scheduled to be in the area. Everyone whom I questioned said that they never saw or heard anything out of the ordinary. I then proceeded and checked on the charts to see if the bed checks were initiated consistently after every hour on the hour where I found out that Elena had initialed the bed checks consistently. Not satisfied enough I went on questioning Elena who responded that she did the bed checks on time.

Recommendations

1. Reduce the bed check time

The bed check time initiation should be reduced to 30 minutes. By doing this will enable and help other patients who might fall from the bed as in the case of Mrs. Lopez not to stay on the floor experiencing cold for a long time.

2. Install CCTV cameras

The CCTV cameras should be installs in the rooms where the patients stay. This installation will assist in finding out the real cause of incidents such as the one which occurred with Mrs. Lopez

3. Install biometrics

Biometrics like using of finger prints should be installed in the patient’s room. By installing these biometric in the rooms will verify that the nurse aid checked in at the recommended time hence making them accountable of any incident that might occur when they are on duty.
