VERY IMPORTANT ASSIGNMENT GUIDELINE TO CAREFULLY FOLLOW
The final task rundown. This task is more about application of the content.
For this task you need to choose two of the four case studies in the appendix of the assessment information guide. You will be presenting two 1250 word case studies.
Each case study will be broken down into 5 elements in which you will present a plan of care:
1. Outline the critical biological, psychological, sexual and social health issues that
need to be addressed.
Given the word limit constraints, you likely will not have the time to go through and provide a thoughtful discussion surrounding the 5 P's. Essentially what we are wanting you to do is when going through the case study, identify and summarise the issues present in relation to those specific areas. This can be done as one paragraph. Think of it as similar to a summary of the key points which need addressing for this person.
In terms of referencing, generally we reference if we need to support our statement/ argument. So if you are identifying something as an issue and then link this to existing knowledge, this will need to be referenced. Here is a hypothetical example:
If Elmo's BMI is 29, that is an issue. We know that is an issue because existing evidence tells us that this falls into the overweight category. So you would write something to the effect of: Elmo's BMI is currently 29 which puts him in the overweight category (Author, 2021).
2. Outline a plan to holistically address/resolve the health problems, providing
reference to the research evidence and authoritative guidelines as
appropriate.
In the form of a paragraph, drawing from what you have identified as health issues you will then develop goals of care as part of a plan. These goals need to be practical and reasonable goals. We want to see your critical thinking here.
I would suggest that you pick key features of your chosen individuals from a physical health perspective. Look at the case study as a whole. For example, is their primary goal to start a family? If so, your goals in your plan would be surrounding what can be done to support this person to start a healthy family.
3. Recommend referrals to allied health or other health service providers, if
appropriate, and outline (based on research evidence or guidelines) how those
professionals might respond.
In the form of a paragraph you will provide some suggestions of who we would refer to and then how the service/ professional we have referred them to might respond. In the case of hypothetical Elmo, we would be wanting to reduce his BMI to improve his physical health. Allied health professionals who might help with this specific aim might include an exercise physiologist or a dietician. Hopefully Elmo also will be involved with his GP as well so a holistic approach across the MDT will be applied. Then we would explain what role those professionals would have in Elmo's care.
4. Recommend one or more community resources or agencies which the person
might access to assist them in improving their health.
This section need only be brief, but have a look around and see what community organisations are out there to provide additional support. Are there eating disorder support groups? Men’s sheds, physical activity groups (think back to task one) etc., etc. IN AUSTRALIA
5. Write a brief narrative completing the scenario, describing how you anticipate
the problems will be resolved (including time frames).
In this section you are going to more or less be writing a summary of what you would expect. This should be realistic. If you have looking at changing behaviours, saying 1 week is not a reasonable time frame. It would also be worth considering any challenges one may face here as well.
The finer details:
We do not mind how you choose to set this out. You can use headings if you wish, you can intertwine the elements within each care plan goal. Provided we are the readers can logically follow your train of thought, that is what matters.
Yes, you need to have references throughout the assessment.
No, you cannot write in the first person (no “I think we should……”).
No, you cannot embellish the case study and add bits in.
As for the word count, my suggestion would be given that it is 2500 words, split each case study into two. Case study one = 1100 words, Case study two = 1100 words, references = 300 words. Given that you have 5 areas which need discussing for each case study break them down in accordance with their weighting on the marking rubric.
Question 1= 30%
Question 2 = 40%
Question 3= 10%
Question 4= 10%
Question 5 = 10%
