IEP Assignment
Directions: Today you begin your IEP project. From the syllabus you will note the piece that
says, "Individualized Education Plan Goals and Objectives: You have received data for
analysis regarding a ‘present level’ of a child’s educational performance. From the
information, you will develop a minimum of four goals and at least two measurable
objectives for each goal."
Please review the Quality Goal Writing Power Point found in modules below this
assignment.
You are completing this project for a 5th grader getting ready to transition to 6th grade. The
child was 8 years old in 2011 when some testing occurred. Please use the attached Missouri
State Sample IEP goal/objective sheet for your assignment.
As part of an attached write-up, explain your thinking on each goal by answering the
following: (This will be answered on a separate document)
1) Why did you chose this as a goal?
2) What is the baseline of the child?
3) What supports/accommodations and modifications are needed for progress on this goal?
4) Who will be working on this goal with the student (service provider)?
5) What additional materials might be used to help the student reach this goal?

Write up in APA format 4 points
Goals 2 points per part (name, direction, area, action, conditions, measurement, frequency), x
7 parts x 4 goals= 56 points
Write up 2 points per question x 4 goals x 5 questoins= 40 points
