What problems need solved ?
A community health worker (CHW) is a member of a community who is chosen by community members or organizations to provide basic health and medical care within their community.
• Currently CHWs are tracking the patient information on paper.
• Because of this there is no way to longitudinally track patient information between the CHW check point and visits with providers.

Professional Services Includes customization of layout and modules, along with upkeep of the application.
Licensing The rights to own the data collected from I-HOPE
Subscription A yearly fee to each CHW location utilizing the application, in addition to data storage.
Unit sales Availability for the patient to have a copy of their own records
