I-Hope Revenue Opportunity Pitch Presentation

PRESENTED BY TEAM 7: Lesley Parker Jessica Spano Adrian Medina Nikhil Natesh

What problems need solved ?

A community health worker (CHW) is a member of a community who is chosen by community members or organizations to provide basic health and medical care within their community.

• Currently CHWs are tracking the patient information on paper.

• Because of this there is no way to longitudinally track patient information between the CHW check point and visits with providers.

How can I-HOPE help? ‘Improved Health Outcomes via

Population Engagement’

● Remove paper from data collection ● Dashboard with various clinics ● Multiple activities to choose ● Efficient & simple process ● Record & retain information ● Synchronize disparate data across

multiple CHW clinics ● Ability to integrate into existing

EHRs
