Weight Loss/Eating Disorder Facility Questionnaire

This questionnaire is intended to evaluate various community weight loss or eating disorder facilities in different parts of the United States. The areas covered in this questionnaire will address the physical, psychological, intellectual, social, and spiritual well being of the patients. This questionnaire will help evaluate the programs value and provide advice to improve on any facility limitations.

Physical Area:

1. Does the program discuss/promote physical activity?   YES     NO    N/A

2. If the program promotes physical activity, does it discuss the impact on other co-morbidities like obesity, diabetes, cardiovascular disease, and cancer?    YES    NO   N/A

3. Does the program account for and/or educate about how to fit exercise into ones current life style? YES NO N/A

4. Is the program equipped to deal with participants that may have physical limitations without excluding or discrimination against them?   YES  NO   N/A

5. Does the program monitor use anthropometric data to assess participant’s needs and track progress throughout the program?  YES NO N/A

6. Does the facility accommodate overweight and obese individuals with bariatric chairs in the waiting areas? YES NO N/A

Psychological Area:
