Stakeholder Identification:

To identify the three stakeholders who will contribute to the success of my project proposal by becoming members of my project team, I used the following technique:

1. Brainstorming: I used brainstorming with a group of healthcare professionals to identify potential stakeholders that could contribute to my project. The group included registered nurses, physicians, allied health professionals, patients, and family members.

Based on this technique, I identified the following three stakeholders:

1. RN1 - Registered Nurse with 10 years of experience in the Medical-Surgical unit: RN1 has experience in implementing quality improvement projects and has expertise in data collection and analysis. RN1 will have a key role in leading the data collection and analysis process for the project.

2. MD1 - Physician with 15 years of experience in the hospital: MD1 has experience in patient care and will provide valuable input on the patient care processes involved in the project. MD1 will have a key role in reviewing the clinical practices to identify areas for improvement.

3. PT1 - Patient with experience in the Medical-Surgical unit: PT1 is a recent patient who has firsthand experience of the care provided on the unit and can provide valuable feedback on the patient experience. PT1 will have a key role in providing feedback on the patient experience and helping to identify areas for improvement.

These three stakeholders assisted with the Affinity Analysis, SWOT, and Impact analysis by providing their unique perspectives on the project. RN1 provided insight into the data collection and analysis process, MD1 provided input on clinical practices, and PT1 provided feedback on the patient experience.

Needs Assessment:

To determine the need for a HIP, I used the following process:

1. Brainstorming: I used brainstorming to capture ideas from stakeholders. The group included registered nurses, physicians, allied health professionals, patients, and family members.

2. Affinity Analysis: I used an affinity analysis to organize the ideas into themes. This process involved grouping similar ideas together and identifying key themes.

3. Consensus Building: I used a consensus-building process to reach an agreement on the need for a HIP. This process involved presenting the key themes identified through the affinity analysis and soliciting feedback from the stakeholders to determine the priority area for improvement.

Problem Identification:

The problem that will be the focus of my project is the high rate of patient falls in the Medical-Surgical unit. The significance of this project to the target population is that patient falls can lead to serious injury and can prolong hospitalization. The significance of this project to the organization is that patient falls can result in increased healthcare costs and can lead to negative patient outcomes. The significance of this project to nursing practice is that reducing patient falls can improve patient safety and can lead to increased job satisfaction for healthcare professionals.

Problem Description:

The high rate of patient falls in the Medical-Surgical unit has been attributed to several factors, including inadequate staffing levels, poor lighting, and patient care processes that are not standardized. These falls have resulted in serious injuries to patients, longer hospital stays, and increased healthcare costs. The project will aim to reduce patient falls by implementing evidence-based practices and improving patient safety processes.

Impact Analysis:

The Impact Analysis Template that I completed during my CPE is attached as Appendix A. Based on the impact analysis, the project has an impact ratio of 1.8, which indicates that the benefits of the project outweigh the risks. This indicates that the project is feasible for implementation.

SWOT Analysis:

The SWOT Analysis Template that I completed during my CPE is attached as Appendix B. Based on the SWOT analysis, the organization has strengths in the areas of skilled healthcare professionals and a supportive organizational culture. The organization has opportunities for improvement in the areas of staffing and patient safety processes. Overall, the organization is ready for the implementation
