Activity:
· Revisit your HW#2 where you developed a heart disease prevention program using the Transtheoretical Model for women in the three poorest zip codes in your city. Fill out the logic model template for this program. In order to complete all aspects of the logic model, you will likely need to expand upon your original program. An example of a logic model is posted on Blackboard. Please upload your logic model in the space designated as “Homework #5 Logic Model Upload.”

· Over the course of this semester, you learned about several different theories, models and approaches to behavior change. What challenges exist when attempting to translate these approaches into practice at the population-level? What strategies can public health professionals use to overcome these challenges? Use your textbook, other assigned readings or additional resources in your response.

Applied Exercise:
1. You work at a local non-profit organization aimed at reducing cardiovascular disease among women living in the three poorest zip codes of your city. Your current funding is running out and if you don’t successfully design a fundable program, you will likely lose your job! The funding agency has put out a request for proposals (RFP) for up to $150,000 per year for three years. If you get this grant, you will get to keep your job for three years. You meet with your supervisor and she ask you to draft up a proposal using the Transtheoretical Model as the theoretical foundation of the programming that will be proposed.
a. Please design the questionnaire that will be used to “sort” people into the various stages of the model.
This questionnaire aims to measure the readiness of the participants to take major preventive measures to reduce the risk of cardiovascular disease by exercising, eating healthy, and quit smoking. This questionnaire should be fully filled by participants in the program and based on their answers they are going to be placed into various stages.

b. What format will you use to deliver your intervention? Face-to-face? Social media? Computer-based online learning module? Text-messaging? Or other? Why did you select this method for program delivery?
In my proposal, I will use the face to face format to deliver my message to participants. This format is reliable as it’s cost effective. You can get first-hand information from individual’s views. This format is also reliable to a large congregation of a target group. It also does not require any level of literacy such as in computer operations.
c. What is your strategy for reaching members of the target population and getting them to participate in your program?
In order to apply this intervention to women who live in poorest zip codes of the city, a systematic approach for reaching and recruiting the at-risk individuals is required to be used. The members of the target population will be reached by phone calls or personal letters to get them participate in the program. Also, I will try to use incentives to attract more participants.
d. How would you measure the success of your program?
I will get the participants to do the same questionnaire after six months. Then, I will measure the level of program success based on the result obtained and a number of participants in the program. The more positive feedback obtained from the program will indicate the success of the program. This is also the same with the higher number of participants in the program. With attention to any progress made by participants, even one stage progression will be considered a success for the participants themselves as well as the program.
