Hazard Vulnerability Analysis - Instructions

Hazard Vulnerability Analysis (HVA) 

Conduct an HVA using the HVA Tool found in the Information Governance Project  folder. A similar spreadsheet of this file can be found in your text Table 13.3 on page 249-250.  

You should be able to download the tool provided in this folder; if not go to the web page,    www.calhospitalprepare.org/hazard-vulnerability-analysis   (you may need to copy and paste the address in the address bar) select Healthcare Emergency Mgt.

From the Table of Contents (on left) - Choose Hazard Vulnerability Analysis.

At the bottom of that page is a link to download the Kaiser Permanente HVA Tool (in your IG Project folder).

The spreadsheet has six tabs at the bottom.  Start with:

· Instructions

· Naturally Occurring Events

· Technological Events

· Human Related Events

· Events Involving Hazardous Materials

· Summary

You will need to evaluate a place that you frequent.   Suggestions are your workplace, home, favorite shopping store, etc.

· Take the form and score each area.

· Summarize by identifying the likely hazards you would need to address in an emergency operations plan.

· Fill in the electronic spreadsheet format and it will automatically calculate the risk impact for you. 

Turn your completed Summary Sheet into the Dropbox before the end of Week 15.
