Week 5 Assignment This assignment asked that you read the case in the instructions and then prepare a 4 page APA style paper in response to three questions about the case.

Each organization should assess which quality improvement

methods are best suited to their operations

The patient should be the center of quality improvement efforts

Why it matters Benchmarking has one purpose, and it is one of the most

important in the CQI process, which is to measure progress

Without a benchmark we have nothing to compare our results

to. If you were told that 10,000 patients visited the facility in

the past 6 months what would that tell you? Nothing. You

have no benchmark to compare it to. Is it high? Is it low? Is it

average? You cannot make sense of data without a

benchmark.

One of the biggest responses to quality issues was from the

Institute of Medicine. The IoM implemented six aims for

quality improvement that they want to be implemented

throughout the entire healthcare system. Patient-

centeredness is the pinnacle of quality of improvement as all

other aims contribute to this goal.

improvement. You will also be exploring various CQI tools such as the Six

Sigma, Pareto Diagrams, and Histograms.

to review this material again as it relevant to our discussion.

Summarize the risk management models

There are a lot, but there are some common elements in all of the risk management models. This includes:

▹ Identifying and categorizing potential risks and threats to success ▹ Assessing the impact of the risk ▹ Determining the likelihood of a particular risk occurring ▹ Implementing strategies to reduce the likelihood and impact of the risk

Examine the role information technology in health care

▹ Improving quality – we can use technology to identify trends or anomalies that can used as benchmarks to improve quality

▹ Recordkeeping – as you likely know, the EHR is replacing the paper record. ▹ Coordination of care – technology allows providers to communicate faster and more

efficiently

Describe the counter measures used to reduce medical errors

▹ Checklists – it is easy to miss steps in a process and using a simple checklist can help to reduce the chance of medical errors.

▹ Patient involvement – involving the patient can help to reduce medical errors. For example, a surgeon may ask a patient to mark which limb they should be operating on.

▹ CQI – CQI is a way to reduce medical errors as if our processes are efficient as possible and we reduce the number of steps needed to achieve an outcome we reduce the chance of an error.
