Opioid Usage Support Expeditionary Team and Equipment

In the State of Maryland, opioid fatalities saw a 14.8 percent increase in the first 6 months of 2018. Baltimore, considered the “epicenter” of this crisis, had 483 opioid overdose deaths during the same period (McDaniels, 2018). Of these deaths, many are preventable with intervention through the injection of Naloxone. Naloxone or brand name Narcan can be quickly and safely injected into someone who is experiencing an overdose on opioids (Davidson PJ, 2002).

Across the world, in 90 sites, there are safe injection facilities that are legal spaces where individuals who choose to inject opioid drugs can bring their pre-obtained opioids and inject them in a clean facility, with safe needle use practices, under the supervision of licensed healthcare providers. These providers are able to intervene in the event of an overdose.

In early 2019, The City of Baltimore convened a panel of city officials, public health and policy experts to address the climbing death rate associated with opioid drug overdoses. This panel ask experts in the harm reduction and substance use services to devise a systematic plan to deal with this crisis. Based on research from other models around the world, our company, Medicicom International (Non-Profit), proposed the systematic development and implementation of a Supervised Injection Mobile Site Facility.

Purpose for the project

The purpose of the Supervised Opioid Usage Support Expeditionary Team equipped with a Recreational Vehicle (RV) type vehicle is to provide a legally sanctioned place where opioid users may safely inject in a manner that will reduce harm associated with opioid drug usage.

Included:

· Decrease overdose

· Decreased healthcare costs

· Decreased incidences of HIV and Hepatitis

· Increase referral to detox facilities

· Improved overall health

· Decrease public exposure to illicit drug use

Project Scope Statement

With highly trained personal, healthcare equipment and medication Supervised Opioid Usage Support Expeditionary Team will provide a safe environment where people whom use of illicit opioids can inject. Within the vehicle will be the availability of 3-4 clean rooms, clean needles, under the direct supervision of trained healthcare professionals. These professionals are trained to work in this unique environment, with this unique population and capable of recognizing and intervening when a potential deadly overdose occurs. The operation schedule will be a 24/7 service. There will be 2 healthcare professionals trained to manage this unique population of opioid users on staff at all times. There will be access to service referrals including detox facilities, social services and primary care. This facility is not designed to provide service outside the scope of the project.

Project requirements

Medicicom International (Non-Profit) will ensure the supervised injection facility is in compliance with all health and city codes throughout the entire lifespan of the project. Specifically, the facility will be in compliance with the all following codes (Consulting-Specifying Engineer, 2014) and Baltimore City Department of Housing and Community (BDHCD, 2019):

· ASHRAE 90.1

· FGI Guidelines for Design and Construction of Hospitals and Outpatient Facilities

· IMC

· ASHRAE 62.1

· ASHRAE HVAC Design Manual for Hospitals and Clinics

· Facility Guidelines Institute (FGI)

· International Code Council (ICC) – International Building Code

· NFPA 99

Additionally, Medicicom International (Non-Profit) is dedicated to meeting the following internal requirements:

· The project must stay within the City of Baltimore’s designated budget

· Materials chosen must be medical grade or suitable for medical setting

· All materials will be sourced and secured at the best price within reason

· All tasks must be completed within the designated timeline

Milestone schedule (summary)

· The Project’s

· Locations: Identifying the locations of the project, to include the neighborhood and surrounding areas will enable the development of a schedule during operations, and it will alert the residents of the area of the future services available to them.

· Staff: Identifying, hiring, and training the right staff will ensure the project is accomplished. With a well-built team, anything can be accomplished. Selecting the team will enable the team to schedule training, re-certifications, and allow for the workers to plan ahead for their personal schedules in order to ensure they take care of themselves and their families.

· Equipment: Identification of equipment, acquisitions and procurement of items is very important. Without the right tools, the staff will not be enabled to conduct their job properly. Lastly, a life-cycle management program will be established for major items once received. In example, how often will tires have to be changed on the recreational vehicles?

· The Day Project Operation Begins: The first day of operation will be the main test for the team, the leadership, and all parties involved. With the feedback received and problems encountered, the team will develop and improve on their skills. Therefore, the first day of opening will have major visibility by Champions supporting the event, local citizens, it will garnish the attention of the media, and may receive VIP visits. Therefore, some planning will have to go into the first day this project opens its doors officially.
