Functional Assessment Assignment

For this assignment you will be using the three assessment tools below on someone over the age of 65, or someone you believe has a “disability” of some kind, in order to assess their functional status. Katz Activities of Daily Living

1. Lawton Instrumental Activities of Daily Living Scale

2. Geriatric Depression Scale (Short Form)  

Step 1: Interview your “client” and gather information regarding their brief health history (age, gender, social history (marital status, occupation, smoking/drinking/drugs), current medications – name only, and brief past medical history; NO review of systems).

Step 2: For each assessment tool:

1) Write a short paragraph summary of what the tool assesses

2) What were your “client’s” results? (Do not include the actual forms).

Step 3: Final paragraph: Write an overall summary of your “client’s” functional status based on the results of the three survey tools.

Submit the paper into the drop box in module 7.

Use APA format including cover page and reference page as well as in text citation of your textbook) Expectation is that the assignment will be no more than 1-2 pages. The paper should be in paragraph format – not lists or bullets. You should in text cite the textbook when you write about the different scales. Rubric is below.
