Formative Assessment: Assignment Plan Template

Dear Student

Please complete the assignment plan and submit it by 26/10/16 via Turnitin. Verbal feedback will be provided during subsequent tutorials. Please ensure that you refer to the learning outcomes as you complete this plan. In black, you have the guidelines provided and in red some suggested points which you may add to your plan.

Please remember that you are required to submit a plan, not a draft (drafts will not be considered).

Part 1 - Introduction

You are required to write a generic introduction in which you will present a critical reflection on the relevance of mental health as a subject. You are also required to critically discuss the historical and contemporary approaches in mental health.

In this part, please list as bullet points the main ideas, concepts and definitions you are planning to expand in your final assignment. Please remember to add the reference to the quotes you select. You can also use the ideas you have added to Task 2 (Defining mental health). Please remember that you have to list historical and contemporary aspects in mental health.

Part 2 - Case study (specific)

In this section, please specify the case study you have selected (case study 1, 2 or 3) – you do not need to add the case study to your plan (you will add to your final assignment).

· Analyse the case study, identifying a possible diagnosis from the description provided, justifying reasons for selecting the type of mental health problem, linking with available literature.

· List important factors (bullet points) which will help you to identify a possible diagnosis for the person; add the definition of the selected diagnosis (with the reference); prevalence and incidence rates in the UK; you can also make reference to ICD-10.

· You are also required to discuss the possible forms of treatment and intervention and how the person can be supported, taking into considerations the available mental health services in the U.K..

· List the possible forms of interventions and the services available for the person.

· You are also required to discuss the implications of this diagnosis for the person concerned, including any potential risks.

· After completing Task 3, list the implications of this diagnosis for the person

· List also some of the potential risks for the person (here you can take into considerations risks of not seeking treatment and other factors which will represent a potential risk for the person.

· Critically reflect on how current policy and legislation would define best practice to meet the holistic needs of this service user.

· List policies and legislation which could be applicable to this situation.

Part 3 - Reflection

Reflect on how this could develop and inform your practice when working and /or caring for individuals with mental health issues.

List points which you will expand in your assignment. If you intend to use a specific reflective model, please add the reference, and add the format you are going to use. Refer to your reflective journal (Task 1).
