I ask that you please reconsider the review findings. This case happened to be my first case as an independent provider and I did not take the time to properly educate and familiarize myself on provider rules and regulations, as I should have. Before starting this case I was told by the consumers’ mother, who had spoken with the RN supervisor, that the 485 had been signed and that I was able to begin providing services. I also made contact with the RN supervisor via telephone to confirm this. She gave me the go ahead and we made arrangements for her to meet me at the consumers’ home to exchange the 485 for payment within the week. The RN supervisor kept making excuses about not being able to meet and eventually stopped responding to my communications. The consumers’ mother told me that the RN supervisor reported to her that her daughter was sick with cancer and that they had been in and out of the hospital and perhaps that was why she was not getting back to me. However, I was not comfortable continuing services. I self reported this issue to the consumers case manager at the time, as soon as I began to doubt that the RN supervisor was going to give me the 485. From there, an investigation was conducted and I submitted all nursing notes and MARS for review. Upon completion of the review I was asked to submit a plan of correction and was told that repayment would not be requested. Unfortunately the majority of my documentation for this case was accidently shredded during a time when I was changing residences. I was however, able to pull up the plan of correction that was sent to me to fill out with the name of the provider monitor who conducted the review on it and I have included it with this letter.
