Explanation

Explain how (your) metric drives improvement opportunities. Consider your issue and how it impacts (drives) improvement opportunities. Be brief and to the point.

Determines quality improvement tool(s) for addressing improvement opportunities. Improvement tools include your framework and the instruments used to implement your intervention, such as a checklist, a protocol, or a policy.

Problem-Solving Framework

Identify a problem-solving framework. Carefully consider your project and intervention. For this performance improvement project, you may need multiple frameworks. Carefully review each framework and its purpose.

Initiative

Develops a concise healthcare quality improvement initiative. This is the how-to section of the paper. Be specific about your intervention and the data. Get to the point in a narrative format of how you will implement the framework and intervention.

Evaluation Method

Determines and describes the evaluation method for the healthcare quality improvement initiative. Refer to Module 6 feedback and the readings in the modules. This evaluation refers to your initiative and the people involved.

Data Collection

Determines data collection for continuous quality monitoring. Data collection tools relate to your intervention and its assessment of success. What data is to be collected? How will the data be collected? Who will collect the data? What timeframe? How will the data be documented?

Conclusion

Summarizes the performance plan and concluding remarks. Recommends ongoing evaluation, issue monitoring, and future adjustments/goals.
