Part 2: Individualized Practicum Learning Objectives

Objective 1: To improve my skills and techniques in psychiatric assessments and evaluations when addressing the health issues of mental health patients. (What is the time frame?) feedback

Planned Activities: To exceed the minimum

required number of patients that should be assesses and evaluated (80) during the practicum for the purpose of enhancing my knowledge about the efficient skills and techniques to be used for similar patients.

(Define exceed, it must be measurable. If your goal is to see 90 then state that. However, you do not need to exceed the required number to reach your goal). Feedback

Mode of Assessment: An assessment of the reliability, validity, standardization, and practicality of the assessment techniques

used is the mode of assessment for this objective (Gold & Frierson, 2017). (The mode of assessment is how do you plan on documenting and providing evidence you met your goal. For example, in this goal you would document at least 80 patient encounters in meditrek). Feedback

PRAC Course Outcome(s) Addressed:

· To enable the interpretation of an individual’s mental health status based on the assessment carried out. (This is not one of the PRAC course outcomes listed in the syllabus and in two previous course announcements explaining that you must connect your goals to these specific outcomes. Please see the syllabus) (Feedback)

Objective 2: To improve my skills of integration of assessment findings and differential diagnosis to identify and prioritize the health

issues of mentally disabled patients. (You want to improve your ability to identify the correct diagnosis and/or prioritize the differential list). Feedback

(What is the time frame?) Feedback

Planned Activities: Consulting my preceptor and conducting research about the effective evidence-based practices for combining and integrating assessment findings and differential diagnosis to be used to identify and prioritize the issues presented by mental health patients (Goolsby & Grubbs, 2018). (You should be conducting comprehensive psych assessments and using that information to develop the diagnosis. You would first want to consider observing your preceptor then conducting one yourself etc....) Feedback

Mode of Assessment: An assessment of how well the findings were utilized to list the possible diagnoses and narrow down to the proper diagnosis of a patient’s disorder is the mode of assessment for this objective (Goolsby & Grubbs, 2018). (Again, the Mode of Assessment is how you will document and provide evidence you met your goal. Where would you do that? How?) Feedback

PRAC Course Outcome(s) Addressed:

To develop a list of effective evidence-based practices for integrating assessment findings and differential diagnosis processes. (This is not one of the course outcomes.) Feedback

·

Objective 3: To improve my skills of how to effectively conduct an assessment of a patient’s response towards the care being provided and utilize the feedback to successfully modify the plans of care. (You will not be developing plans of care for this course. The focus is on assessment and diagnosis. Treatment comes in future courses. Learning to identify the correct assessment tool/scale to aide in diagnosis would be a relevant goal.) Feedback

Planned Activities: This will involve individually monitoring at least 12 patients under care and carrying out an interview to gain feedback about their views on the care. This feedback will then be used to effectively adjust the care plans and monitor that they are successful. (Delete and revise the above goal to address the course learning outcomes.) Feedback

Mode of Assessment: The mode of assessment for this objective is through observation of the patients as well as using the DSM V to guide the decisions made about the care plans (McGuiness et al., 2019). (Where and what are you documenting to provide evidence, you met the goal.) Feedback

PRAC Course Outcome(s) Addressed:

Analyze the patients’ views to come up with adequate information of care plan programs. (This is not a course learning outcome.) Feedback

·

· Accurately modify plan of care for specific patients.
