Transition Care Plan
Family is an essential aspect of society at any given time. That is why many people would
prefer getting attended to by the people they share the same bloodline during need. For instance,
Snyder’s situation proves how the family is vital. She desires to have her family beside her during
her last days on earth, whereby she had already surrendered to the disease and was ready to die at
her home. Therefore, it is all about family because they are among the people who will understand
the feeling you have deep inside and act according to how you would prefer to get followed. At
some point, through the closest people you know from the shared bloodline, their Presence would
help boost your psychological stability during medication and sometimes improve recovery due to
the present contact (Christians, 2021). Usually, one believes that having their people around them
during need is the perfect solution towards providence of specific basic needs and wants as the
patient would suggest. That is why Snyder wanted to be taken home and die peacefully at the
conform of her settings alongside her family and the hospitality services she prefers from a specific
Jewish center.
Elements and information needed for quality transition for improved patient outcomes.
Usually, as depicted from the case study, the patient was in a condition that had forced her
to give up. Sadly, there was medical staff around, but the situation could not get solved as one
would expect. Therefore, several practices would be vital in making the entire examination
successful in increasing the quality that would involve better patient outcomes. The first includes
patient education, whereby the client needs to realize that the current medical condition is not the
final aspect. Anything might change at any time, bringing back the lost life. It encourages one to
live rather than die before the actual time comes. Usually, some persons succumb to illnesses due
to anxiety. However, patient education would provide advance directive information leading to
3
changing one’s psychological status, resulting in improved medical conditions. The second
pertains to patient engagement throughout the process. For instance, when any aspect occurs, such
as emergency practices, she should get the detailed information regarding the process, which
should get initiated professionally. The other approach involves incorporating one’ community,
people from the specific bloodline. They are the psychological boosters of the patient under care. It
helps them have hope regarding the situation, hence a promising medical examination throughout
the course. Lastly, some information does not need to reach patients during critical conditions.
They include the rising medical bills, the death of other persons that had been in such conditions,
and how dangerous the current situation might be. It fades their hopes of living a healthy life again,
hence dying before the projected time. Providing positive information helps one adapt to the
situation and accept everything, thus increasing their lifespans. That is why people have been
living with HIV/AIDS for more than thirty years, as others succumb to it within a year. It is all
about what they know regarding the illness and how the medical staff will react to the knowledge
possessed.
Importance of the key elements of Transition Care
It is critical to understand that the method and manner of conveying information to the
patients matter the outcomes. Threatening data usually makes them feel more anxious within the
current situation. It worsens the medical condition hence death. Regarding incomplete and
inaccurate information, there would be aspects of medication errors, longer stays at medical
facilities, increased admission rates, and higher care costs. Therefore, giving the most accurate data
helps understand the current situation and examine the proper ways that may need attention that
leads to improved health. The other importance of the stated elements includes patient education's
power towards recovery. It is among the central focus aiming to influence one's behavior alongside
changing their skills and attitudes towards the medical concern, helping improve their health (Tom
4
& Phang, 2022). Additionally, including one's community can be a care plan concept. The
community or the client's people get considered as persons who understand one's situation and act
as the key players in social responsibilities (Melo & Alves, 2019). Typically, one believes that
being with people they relate to through blood boosts their trust regarding the induced processes.
Importance of effective communications with other health care and community services agencies.
In every situation, the flow of information is among the critical concerns that matter the
practice outcomes. Communicating effectively associates with giving accurate data suitable for
conducting matters according to how they occurred and finding solutions. The first importance
includes proper protection of the clients. Regarding safety, when the exact message and
information gets communicated along the line of duty, the medical examinations alongside issuing
of drugs to the patients will be accurate; for that reason, it will be easier to give the proper
treatment to the patient. The second involves bringing a collaborative practice among professionals
and the community, creating a reliable form of medication, since professionalism, collaboration,
and coordination aids in increased monitoring and examination of patient needs in any given
situation. The third involves saving costs since effective communication aligns with proper
medication, which helps deliver the exemplary practices at the exact time of need. For instance,
there will be a coordinated form of activities within the healthcare facilities when a patient gets
admitted. Therefore, the person responsible for giving drugs to the patient will rely on the data
presented and produced from the lab. If it is accurate, the correct prescriptions will get issued,
saving them time and costs for the affected person. Lastly, effective communication perfects day-
to-day efficiency. It gives the proper updates at the perfect time, improving the chances of having
better patient outcomes. However, if there is ineffective communication during the transition, there
will be increased costs, advanced medical errors, poor collaboration towards helping solve the
5
situation, and lack of efficiency in treatment, and delays in the necessary tests. Due to such
reasons, there will be an inferior patient outcome at the end of the practice, leading to adverse
suffering or even quick deaths.
Barriers to the transfer of accurate patient information from the sending organization to the patient destination.
Poor transfer of information to the intended destination results in poor outcomes. To avoid
such instances, the barriers must get identified and shut. Such barriers include poor technological
use, uncooperating patients, and security concerns. For example, in home care services, when the
correct information gets delayed to reach patients’ homes due to security concerns, there will be
reduced speed in medical examination, limiting the chances of getting accurate results on time.
Regarding patients refusing to corporate, family involvement may have been the course of it all
due to their advice. Some family members discourage the sick from expected services by hiding
the required data from the lab.
Accurate Provider Understanding During Patient Care Transition
The best strategy for ensuring that the destination care provider has an accurate
understanding of continued care involves facilitating proper communication with outpatient
providers. It is vital to understand that having follow-ups regarding the condition of clients after
they leave a facility to understand their current situations that include the entire progress through
treatment engagement, community support, early recovery, and maintenance (Schiavo, 2021). By
such practices, the potential outcomes include;
Reduced risks associated with hospital-acquired conditions.
Boosted continuum care.
Best patient experiences.
Reduced waiting hours.
6
Improved rehabilitation and recovery.
Through such a strategy, one might avoid the main disadvantage of outpatient programs,
involving lacking someone to help the patients at all times during the recovery process (Rauch et
al., 2020). It reduces the chances of getting worried about their health situation when away from
the medical facility. That is the same thing that would have been made available for Mrs. Snyder
after her desire to go home.
Conclusion
In conclusion, there is a need to understand the critical elements for a better medical
transition and their importance, having effective communication, understanding the barriers to the
communicated message, and the best strategy to improve patient outcomes. The key elements
include patient education, which builds a client, making them understand the need for the
medication and how it will be better to get it at the right time. It boosts their confidence and belief
in the suggested procedures. Among the key barriers to effective communication is a technological
failure, which delivers incorrect data, leading to poor results. Lastly, the best strategy to get
involved in the current medical practices should include keeping track and communicating with
outpatient providers. It reduces the anxiety of one feeling that they are lonely. One needs to feel
that someone is watching over them; they must be professionals.
