Advocating for the Nursing Role in Program Design and Implementation
Healthcare program within practice and the costs and projected outcomes
Nursing homes face various challenges when providing quality care and treatments for residents with urinary tract infection (UTI). I have been working long-term care with some facilities, and I have realized that upper corporate management functionality is not the same. In the past three years, I have been working as an infection control nurse. I can tell from experience that the total number of infections recorded every day in the facility mostly comes from the UTIs. As a result, my team and I developed a healthcare program that seeks to control and reduce antibiotic medication errors encountered in the facility. The program results were encouraging because we have seen the number of UTIs reducing to almost zero in the past four months. The program’s primary goal was to prescribe correct antibiotics for our residents by using the tool and removing unnecessary ones. The plan is estimated to cost $100,000 to cater to traveling costs and project implementation and educate the practitioners and staff across the facilities and procurement of necessary materials for every different facility. Target population
The program targets older people in long term care facilities because research has shown that most UTIs cases are experienced in long-term care residents (Mason et al., 2020).
The role of the nurse in providing input for the design of this healthcare program
A nurse will play the role of identifying UTI cases by using the SBAR tool that is analyzing the situation, background, assessment, and Request through urinalysis examination. The program will improve the use of antibiotics by getting rid of inappropriate antibiotics. Acting in an advocate's capacity, I will ensure my elderly population’s needs are met within the nursing facility because research shows long-term facilities are breeding grounds of antibiotics strain resistant, which can be harmful (Bubb et al., 2016). The main focus is to provide our patients with appropriate antibiotics while eliminating inappropriate ones that may become resistant. My input design uses every reported tool kit form of suspected antibiotics and discusses it with the facility’s medical team for a follow-up and containment. My input design will ensure that diagnosed UTI will be assessed, and appropriate interventions are provided as an infection control nurse. For instance, in case a resident presents some signs suspected to be a UTI, a thorough assessment will follow and discuss containment measures before notifying the physicians.
The role of the nurse in healthcare program implementation
During the healthcare program implementation, nurses will play a significant role by ensuring that inappropriate antibiotics are removed from the system since they can become resistant to the patient. In order to attain this goal, nurses must collaborate with other health care professionals to ensure that patients receive desired and required antibiotics. This role will vary between design and implementation. The nurse will first follow a suspected UTI SBAR before the implementing process starts to have the lowest introduced antibiotics criteria.
For example,
· We will not look for signs and symptoms of fever in case a patient does not have an indwelling catheter.
· We will look for fever for a patient with an Indwelling catheter.
The members of a healthcare team that is most needed to implement a program
This healthcare care program is majorly targeting the facility’s practitioners because the ones are prescribing the antibiotics. A policy written in the form of a letter will be sent to every practitioner, informing them about the new clinical guidelines developed within the nursing home about antibiotic prescriptions. The hospital is determined to create protocols used to examine UTIs via urinalysis to enhance antibiotic use by constructing an SBAR tool kit that will allow nurses to comply before notifying physicians. The information shared must be research-based in order to improve the communication and feedback between the nurses and clinicians to improve patient care outcomes. Other members on board with this health care program are the nursing assistant, licensed practical nurses, registered nurses, nursing care team, and nursing directors.
