As part of its ongoing efforts to improve access to health care, General Medical Center is subsidizing the start-up and first year of operations of a new family medicine practice, Park Square Family Medicine. 

As a advanced practice nurses (APN), I have invested a lot of time and money into education  I have gone through dealing with the state laws and nurse practice acts, also gone through the principle of autonomy / collaborative approach of practice with physician 

This is important because it fulfils the requirement of the APN professional business plan requirement and also to have an insight to what it takes to set up a clinic after licensure as an APN with practice privileges.

Our vision is to bring quality and affordable healthcare. The clinic aspires to bring special healthcare services, health educational programs (primary preventive measures) as well as personal health and wellness programs.

The targeted location for the hospital set up will be in Colorado. This location is best since there is a wide range of people and the population as well is fair which implies that the hospital will have patients in regular basis which further ensure that the hospital will have a continuous flow of income. Before choosing the location, a thorough research was conducted, and further feasibility studies were conducted so as to be able to penetrate the available market and most importantly become the preferred choice.

Therefore, they have the best arms in the whole of Colorado. Moreover, the hospital will be operating for twenty-four hours a day unlike other hospitals. Also, the clinic has a strong ethos of openness, sharing and commitment to increasing patient confidence Patients wanting to get involved Local charities willing to participate. The weaknesses limited is that it is just starting out and therefore may not have sufficient required finance which will sustain the kind of the publicity which is intended so that the business can be well known and also, Staff is not clear of their role in the patient relationship

One of the opportunities is that there is an active volunteer committee willing to plan and organize events. Patients who are active in the clinic’s Patient Participation Project can be asked for their opinions and suggestions. Head Nurse is willing flex clinic times to free up clinical staff time. Also, The opportunities which are available to the hospital are that the people not only go to the hospitals when they are sick, rather, they also go on someday for a routine checkup which id continuous practice for the residence. One of the major threats to the Goldenberg hospital is the economic downturn as well as the unfavorable government policies. The economic downturn will indubitably affect the purchasing power of the clients and thus will perceive check up as they used to. Furthermore, Confidentiality is at risk

Advertisements will also be done by use of media such as the local television and the local radio stations. Moreover, the hospital will be advertised on the yellow pages and leverage on the internet so that it can promote the hospital.

Budget Plan

The most important factor for the financial plan is collection of receivables

It is assumed that there is a strong economy, without major recession.

It is also assumed that there are no significant unforeseen changes in the federal policy dictating Medicare and Medicaid reimbursement.

The Break-even Analysis below is based on monthly fixed costs and an Average Per Unit Variable Cost. This assumption about cost of sales may at first look low, but in our service-based business, payroll is included with other operating expenses in our fixed monthly amounts, so the variable costs relate to the only other cost of service provided: mileage to and from service locations.

Expenses and Revenue

The profit and loss statement will the increasing ability to cover the expenses of the business over the first year.

Although it will operate at a loss for much of this year, the cash balance will be maintained by subsidies from General Medical.

It is estimated that start-up expenses will be $22,732. This amount of money will be used to purchase office equipment, medical supplies, furniture, stationary, and other start-up expenses. This amount, and a starting cash balance of $10,000, will be financed by General Medical Center.

In addition, we are in the process of acquiring a building worth $225,000 with a 15 year mortgage, which will be repaid from the company's cash flows.

The start-up requirements, with the exception of the building loan, are to be financed by General Medical Center. This is in addition to the $288,000 they will be providing over a 12 month period as expense subsidies in the first year, and the physician's salary for the first year.

The purchase of the building will be with a 15-year mortgage (listed under Long-term Liabilities). This loan will be repaid from the clinic's cash flows and guaranteed with his personal assets.

Breakeven Analysis

The Break-even Analysis shows that in the first year (with no physician salary). We will pass this mark by the sixth month.

However, in the second and third year, with increased expenses including the physician's salary, and increased patient load, we will need to bring in roughly $21,400 per month to break even. We do not anticipate reaching this level of patient care payment until the middle of the fourth year, which will end with a modest profit.

On termination of his/her employment the Nurse must return all notes, memoranda or other documents concerning the Practice and/or any patients which he/she acquired, received or made during his/her employment together with all other property belonging to the company. The Nurse shall during and after his employment respect patient confidentiality in relation to all patients of the Practice

PENSION

The pension scheme provisions should be set out herein. If there is no pension scheme then a statement to that effect should be made together with a statement saying that if the employee wishes to enter into a PRSA scheme the Practice shall facilitate this by allowing appropriate payments from her salary to be made to the scheme by credit transfer
