ASSIGNMENT FOR TRUST

Client John Smith makes an appointment with the firm that you are working for in

order to prepare a Revocable Trust. He wants to transfer his home to a revocable

Trust in which he makes his children, John Smith 19, Christopher Smith 16 and

Jane Smith 12, his beneficiaries. He wants to distribute the home to them per

stirpes when they each reach 21 years of age.

He will retain the right to use his home as his homestead while he lives.

He will be the initial trustee and his brother Thomas Smith will be his successor

trustee.
