· Identify a target population and give reasons (from the literature) why the population you chose is “at-risk.”
Part 2 - Program Proposal

Part 4 -Business Model

· Display and describe your organizational chart.
Part 5 - Mission Statement/Goals and Objectives
· 1 mission statement
· Up to 5 goals, no less than 3 (should be based on the first year of the program)
· No less than 1 objective per goal
· Each objective must be measurable (such as “By the end of the first year, I will have 50 clients.”)

· What is your messaging? Why? (unique selling proposition - what makes you different from your competitors?)
Part 8 - First Year Pro Forma
· Key assumptions (what things must happen in order for your proforma to be logical). Example: Secured 50k in funding and gained 50 new members in a pre-opening promotional campaign.
· What is your initial capital outlay? (cost of setting up the business including cost of equipment, buildout, marketing, app development, etc.)
· Display monthly and annual expenses and all sources of income (i.e. group training, 1 on 1 training, products, etc.)
· What is the tax status of your employees (W2, 1099 subcontractor, salary, commission)
· After employees (and payroll taxes are paid) what is your monthly and annual net profit
*W2 employees pay 7.65% (FICA tax rate 15.3%) and the business pays 7.65% (FICA tax rate) on that employee.
**1099 subcontractors must pay the full 15.3% of their total income minus itemized deductions. The business pays no FICA on 1099 employees, but there are strict rules on subcontractor status.
***Salary employees are paid a flat annual rate spread equally over pay periods. This compensation method makes it easier to calculate annual payroll expenses.
**** Commissioned employees are paid a percentage of the income they generate. This is common in fitness centers to incentivize trainers to get referrals.
Part 9 - Funding Plan
· Will you need working capital (cash) to set up your business?
· What sources of funding would you pursue? (personal loans, GoFundMe, donations, etc.)
· What barriers might you anticipate?
