Summary of strengths:

I can recognize clinical signs and symptoms of the majority of psychiatric illnesses, differentiate between pathophysiological conditions, and interpret a mental status exam too. Moreover, I can diagnose according to DSM-5 criteria based on the given assessment information. In my pharmacotherapeutic skills, I can document, evaluate patient response, and modify the plan with a little help. In psychotherapeutic treatment planning my strength lies in recognizing the concepts of therapeutic modalities across the lifespan. I can also confidently collaborate with multidisciplinary teams to improve clinical practice in mental health services. I can also select the appropriate screening instrument for any specific clinical situation

Opportunities for growth:

I have an opportunity for growth in using the SMART strategy in my self-assessment skills. I can also improve in interpreting the mental status exam with a little help. I can also improve in the assessment of adverse reactions in my pharmacotherapeutic skills. I have space for growth in age-appropriate plan of care and psychotherapeutic counseling techniques with individuals or families. I can also do a better job in evaluating outcomes of the practicum goals and modify plans accordingly.

Goal 1: Improve Comprehensive Psychiatric Assessment Skills

Objective 1.1: Conduct a minimum of 10 full psychiatric evaluations under preceptor supervision by midterm.

Objective 1.2: Accurately use DSM-5-TR criteria to formulate diagnoses in at least 80% of patient cases by the end of the practicum.

Objective 1.3: Integrate standardized screening tools (e.g., PHQ-9, GAD-7) in 100% of appropriate cases.

Goal 2: Improve Foundational Psychotherapy and Counseling Skills

Objectives:

2.1 Reflect or co-lead at least 5 psychotherapy sessions (individual, family, or group) by week 9.

2.2 Try at least 2 therapeutic modalities (e.g., CBT, DBT, MI) with patients and document their effectiveness in session notes.

2.3 Ponder on therapeutic interactions weekly in a clinical journal and discuss improvements with your preceptor.

Goal 3: Enhance expertise in Risk Assessment and Safety Planning

Objectives:

3.1 Conduct a minimum of 5 structured risk assessments (e.g., suicide, homicide, self-harm) using validated tools during the practicum.

3.2 Create a collaborative safety plan for at least 3 high-risk patients in coordination with the preceptor.

3.3 Attend or review 1 institutional training on crisis intervention or safety protocols during the first 4 weeks of the practicum.

Goal 4: Improve Multidisciplinary Communication and Collaboration

Objectives:

4.1 Participate in weekly interdisciplinary team meetings, contributing at least one patient case per meeting.

4.2 Communicate clinical findings and care recommendations to psychiatrists, social workers, and other team members in at least 5 cases.

4.3 Document all collaborative care efforts in the EMR, ensuring alignment with treatment goals.
