PART #1. 5s methodology (30 points)

Select the area or workplace you would like to improve.

1. Identify problems in the selected area that you believe can be solved by implementing 5s procedure. List the problems.

2. Submit the layout of the selected place and show a picture as evidence of the current problems/situation you may have. This area could be your own personal closet, your kitchen cabinets, your office, your workplace area, etc. Layout could be submitted using a CAD software or Microsoft word, or picture.

Evaluate your current workplace area by using the 5s audit checklist. Make a 5s checklist to evaluate the workplace organization. Your 5s audit checklist should include a radar graph indicating the evaluation of the workplace score. See following link to build a radar graph. You can also look for more resources/ videos on building radar graphs. You can use the 5s checklist Excel file to evaluate your area. You can use the same information shown in the Excel checklist if applicable to your area. If not, you need then come out with your own list of problems. In the Excel checklist, when you see that the highest score is 5, it means that there are zero problems in that particular subject.

PART #2 Kaizen (20 points)

1. Write a page essay about the relationship between Kaizen and lean manufacturing. Use times roman 1.5 space between lines.

2. Cite at least two sources of information. Include a list of references

PART #3 DFM (20 points)

1. Write a page, 1.5 space on the theory of design for manufacturing such as definition, advantages, disadvantages, talk about the implementation procedure, and talk about challenges in implementing it.

2. Select at least two sources of information. Cite your references in your document and include a list of references.

PART #4 FMEA (30 points)

Application of FMEA (Failure Mode Effect and Analysis) to design a product.

Directions:

You are asked to create your own case study by selecting a product you are familiar with. You are not allowed to use the same product as your classmates or the same product you used for homework or from Internet. Hint: automobile part, household product, workplace product, etc.

Answer the following questions:

Exam #1 GRADING RUBRIC (100 points)

85-100 points

Criteria

· The exam is organized around a critically considered, original and well-documented problem. The content of the exam is accurate. Points raised are factually grounded.

· The exam has a cover-page, a table of content, a body (answers to the questions), a bibliography and an appendix; charts, tables and graphs are created using Excel or Power Point, and displays original research.  Useful illustrations may also be included.  The exam is well written.

70- 84 points

Criteria

· The exam is organized around a coherent and well-documented topic, with a body (answers to the questions)

· The exam may be somewhat limited or lacking in critical thinking and original ideas or observations.

· The exam has a cover-page, a manuscript body, a references section (bibliography) and an appendix. All charts, tables, and graphs (included where appropriate or required) are created using Excel or PowerPoint, display original research, and are labeled and referenced (if they reprint existing data published elsewhere). Relevant illustrations may or may not be included. The exam is written in accordance with the appropriate style guide.

50-69 points

Criteria

· The exam does not address a body (answers to the questions) and is poorly written. It is poorly organized and lacks a coherent topic.

· The exam demonstrates a lack of understanding and/or a lack of effort in exploring the topic.

· The exam has a body missing a title page, Charts, tables, or graphs are missing where they are appropriate or required, as are relevant illustrations. There are problems with the formatting of the exam according to the appropriate style guide.

30-49 points

Criteria

· The exam is lacking of organization and content. It is very poorly written. Little understanding or effort was put into this exam. It is missing fundamental sections such as a title page, and a body (answers to the questions). There are serious problems with the formatting of the exam.

Zero Points

· The exam is plagiarized (or otherwise in violation of the Academic Behavior Code), or not submitted at all.
