Milestone: Evaluation Paper ​The primary purpose of this milestone was to develop an evaluation plan for a healthcare program regarding healthcare disparities and inequalities. Also, the evaluation process demonstrated measurable reductions of inequities in diverse populations of patient. The context of this milestone included the proposed program’s financial elements and approaches of evaluation. Financial Aspects ​One of the specific resources suggested for the program included the National Healthcare Quality and Disparities Report. The report was crucial for the program as it aided in summarizing the quality and access of health care across numerous ethnic, income groups, and racial and other significant populations such as residents of the communities. Another essential resource that was suggested included the Health Disparities and Inequalities Report-United States (2013)(AHRQ, 2020). This helped the evaluation team in consolidating the most latest national data present on disparities in morbidity, mortality, and health care access, social determinants of critical health issues, behavioral risk factors and preventive health services across the United States indicators. Moreover, Mapping Medicare Disparities was suggested to address Disparities and Quality Improvement.  ​The project was based on a general public health concern. As such, the team made concessions to the government since the programs could not fit into the community’s budget. Health care disparities and inequalities among communities is a responsibility that the government would take accountability in terms of financing. It was essential for the government to offer the necessary resources for free to reduce the budget for community.  Evaluation ​Benchmarks will be used to measure and evaluate the proposed program effectiveness in the communities. Here, the initial data and the current one regarding the cases of health care disparities and inequalities will be compared to determine the differences. Besides, the team will use Patient Experience to measure the outcome experiences of different patients especially those who had previously complaint about health disparities. ​One of the tools the team will use to measure the effectiveness of the program includes Continuity of care between care Levels. This will allow the team to record the consistency of care given to diverse populations over time. Also, Care Transition Measure will help to measure the transition of care in the diverse communities’ residents (Melle et al., 2018).This will enable the team to determine whether healthcare transitions are conducted effectively on some group of individuals than they are done on other ethnic groups. Moreover, B-prepared is another tool that the team will use to measure how different patients from the diverse communities’ population are discharged to determine whether there will be any action of disparity and inequality in different healthcare organizations. ​The program will go to an extent of educating the healthcare providers on the importance of heath care equality. Helping such health care professional to understand the concept of disparities and inequalities will promote the humanity and effective care-giving to patients regardless of their ethnicity or background in the healthcare services. Furthermore, the program will go to an extent of educating the patients about their health care rights and to encourage them to report any disparities’ or inequalities-related cases to the authorities immediately (Melleet al., 2018). As a result, this will ensure healthcare professionals takes on their duties according to the set healthcare services regulations especially those related to disparities and inequalities by the government (Yearby, 2018). This will help reduce the number of cases related to healthcare disparities and inequalities. Also, through the provided resources by the government, then team will follow up with the services offered by the healthcare centers that had most healthcare disparities and inequalities related cases.  References AHRQ.
