Evaluation Method  #2: SELF-EVALUATION

Paper must be in APA format.

Limit: 3 pages, including the Self-Evaluation form. 

Use the Formative Self-Evaluation Form checklist, along with the teamwork-related articles on Canvas and the Model for Improvement (see pages 200 – 202 in Nash book) to conduct a self-evaluation and prepare an action plan to address deficiencies in your role as a team member for this course. Your action plan should include both the actions you will personally take to address any areas of weakness, as well as your willingness to work together successfully with your team members throughout the semester. After your teams are formed, each team should review this document and discuss how the team can address these concerns early in the semester. While not required, it may be helpful to share the results of your self-evaluation with your teammates to help guide your discussion about roles and responsibilities. Periodically reviewing the document will help prevent problems as each team progresses through the semester.
