Its Week 1: Ready for a Little Gift? Read this and respond for a substantiated post.

Before we begin this week's discussion questions, I want to elaborate a bit on the differences between PI, EBP and Research. I believe there is a bit of confusion out there about the three. Here is how I keep it all straight in my head:

Performance Improvement (PI) (Quality Improvement/QI) is the process of making improvements at the unit level. Your goal is to improve a process that will ultimately improve patient outcomes.

Evidence Based Practice; Let's say you are looking for way to make things better and you do a literature search on the problem you are trying to fix. You come across a wonderful new practice that you currently don't have within your unit/facility/system, and it's baked by research. You implement the new process/initiative, etc... that's EBP. It's the best of the best and backed up by data.

Research: You have an idea to do something for your unit/facility/system. You do a literature review, and you can find nothing like it anywhere!! It just does not seem to exist. You decide to do a study... looking at your data, going through the research process and implement it: collecting data, etc. along the way. Let's say it works!! It's wonderful!! Your initiative or new process, backed up by the research and data you have-is now Evidenced Based Practice and can be implemented by others!

I hope that helps clear up the difference between the three and helps with your literature searches to understand a bit better the type of articles you are looking for and at.

Evidence-based practice (EBP) refers to a method of delivering healthcare that incorporates the best available research, patient care data, professional skills, and patient preferences and values into a problem-solving process (Helbig, 2018). When creating EBP, researchers examine existing studies that nurses employ to give patients safe, high-quality treatment and to enhance outcomes. A nursing practice problem is a situation that arises in the course of nursing practice, such as during a patient assessment, that has to do with how the patient is reacting to a health problem they are going through. This can include the patient's illness's signs and symptoms as well as any difficulty they may have carrying out their daily activities as a result of an extremities fracture. This is a result of the fact that nursing care encompasses the entire person, family, or community. A medical practice issue is focused on a sickness or condition. It detects a condition, looks for causes, determines a prognosis, and recommends a course of therapy. Take a patient who has a wrist fracture as an illustration. A fracture will be set by the doctor, who will also give painkillers. This is an examples of medical practice problems. The nurse will go through the patient's medicines and aftercare instructions. Additionally, the nurse will go over how to care for the splint and may make plans for a visiting RN to assist with daily duties. This is an example of nursing practice problem. Using the evidence-based practice tool PICOT, nurses can recognize Clinical Questions and provide methodical responses (Granger, 2020). Additionally, the PICOT inquiry structure is employed to help guide the process of finding supporting documentation in the literature. It is crucial to speak with a clinical specialist in the field to help develop the question in order to make sure that the PICOT inquiry is based on a nursing concern.  The PICOT question need to be based on an issue that arises frequently in nursing practice, is likely to enhance patient outcomes, and for which there is some research suggesting that a particular intervention might be successful. It is possible to formulate the PICOT question once the issue has been determined. To produce data that is meaningful to nursing practice and that can result in better patient outcomes, it is crucial to make sure the PICOT question is based on a nursing concern. Furthermore, by focusing the PICOT question on a nursing issue, the researcher can benefit from the depth of expertise and experience that nurses have in addressing the issue at hand. This can be very beneficial for both developing a strong research study design and for identifying viable problem-solving approaches.

Evidence-based practice in nursing is an approach that uses scientific evidence and the latest relevant research to make optimal decisions about patient care and deliver care based on clinical expertise (Abu-Baker et al., 2021). This approach is based on evidence-based practices (EBP) that improve outcomes for patients and health care systems, improve patient health and safety, and deliver care efficiently.

The PICOT Questions format addresses a patient population (P), a problem of interest or intervention (I), a comparison group (C), an outcome (O), and a time frame (T) (Helbig, 2018). Questions in this format help generate searches that generate the most relevant and high-quality information about the topic while also helping to reduce the time required to generate these search results.

Nursing practice problems are problems that occur during daily nursing practice. Medical practice problems arise during a patient's disease associated with a medical condition or disease. Most fractures in the elderly are caused by weak bones, such as osteoporosis and falls. When a patient is hospitalized for these diseases, movement is limited, and pressure sores quickly occur. It is because it mainly occurs in patients with diseases or limitations in body movement. In nursing practice, the patient's situation is analyzed to determine the intervention for the appropriate treatment and to select the appropriate nursing intervention. The nurse reduces the patient's pressure sores by turning over the patient every 2 hours. Further, the nurse observes the patient for a period to ensure that the intervention is appropriate.

Post 4:

Evidence-Based Practice can be defined as the integration of clinical expertise, patient values, and the best research evidence into the decision-making process for patient care.  EBP is an approach to healthcare that utilizes the most current research available to improve the health and safety of patients while reducing overall costs and variation in health outcomes. It is applied problem-solving, combining best practices from the latest medical literature with clinical experience and the values and preferences of the patients under treatment. Research consistently demonstrates that EBP improves the delivery of healthcare, strengthens outcomes, diminishes geographical differences in care, and cuts costs. Employing EBP has been found to increase overall job satisfaction, which in turn reduces burnout.

  A nursing practice problem is an issue encountered during nursing practice, such as during a patient assessment, related to the patient's response to a health problem they are experiencing. Nursing practice problems are issues related to the patient’s care, emotions, and attitude towards actual or potential health disorders. However, the medical practice problem is directly related to the diagnosis and identifying symptoms of the patient. the nursing practice problem is related to the patient’s feelings, while the medical practice problem is related to the patient’s real state and physical symptoms. For example, in the care of a patient with a fracture, the doctor will fix the fracture and prescribe analgesics this is the medical problem. The nurse will then review the aftercare and prescription with the patient -comes the nursing problem side.

 In the nursing practice problem, the care needs of the patient from the time they get out of the health care environment to the time they are diagnosed by the doctor to the after-care periods are addressed. A PICOT addresses various elements of nursing care, and hence it should be primarily based on a nursing practice problem.
