OVERALL COMMENTS

Eduardo,

Thank you for submitting the first course assessment of enhancing quality and patient safety outcomes pertaining to medication administration. This assessment is the foundation upon which you will build the additional course assessments. You did an excellent job following the scoring guide criteria and meeting the assessment requirements. I really enjoyed evaluating the assessment and appreciate your hard work and effort. I look forward to reading your next assessment submission. I was wondering, from doing this assessment, have you changed anything in your own practice to ensure quality of care is delivered and that patient safety is of priority? I hope that you can relate the course material and learning activities to real situations and changes in practice. I look forward to seeing how things evolve for you over the course.

If you choose to resubmit your assessment with revisions, please highlight the new/revised content.

Keep up the hard work!

Sincerely,

Dr. Vawter

RUBRICS

CRITERIA 1

Analyze the elements of a successful quality improvement initiative.COMPETENCY

Analyze factors that lead to patient safety risks.

NON_PERFORMANCEDoes not identify factors leading to a specific patient-safety risk focusing on medication administration. BASICIdentifies factors leading to a specific patient-safety risk focusing on medication administration. PROFICIENTExplains factors leading to a specific patient-safety risk focusing on medication administration. DISTINGUISHEDExplains factors leading to a specific patient-safety risk focusing on medication administration. Makes reference to specific data, evidence, or standards illustrate the safety risk.

COMMENTS:

Good identification of the factors to the specific patient-risk of med errors, such as poor communication. To achieve a higher performance score, you can support the patient-risk concern with data or evidence which illustrates or indicates that the concern is an issue in practice. Be sure to support your findings with appropriate in-text citations.

CRITERIA 2

Analyze factors that lead to patient safety risks.COMPETENCY

Analyze the elements of a successful quality improvement initiative.

NON_PERFORMANCEDoes not identify evidence-based and best-practice solutions to improve patient safety focusing on medication administration and reducing costs. BASICIdentifies evidence-based and best-practice solutions to improve patient safety focusing on medication administration and/or discusses reducing costs but not both. PROFICIENTExplains evidence-based and best-practice solutions to improve patient safety focusing on medication administration and reducing costs. DISTINGUISHEDExplains evidence-based and best practice solutions to improve patient safety focusing on medication administration and reducing costs. Makes explicit reference to scholarly or professional resources to support explanation.

COMMENTS:

Thank you for identifying evidence-based and best practice solution, however, a greater explanation of the strategies is required along with a connection to how cost is impacted when there are med errors. Parts of this section were unclear due to lack of citations to support your discussion. For a higher performance level, be sure to support your discussion with relevant literature.

CRITERIA 3

Identify organizational interventions to promote patient safety.COMPETENCY

Explain the nurse's role in coordinating care to enhance quality and reduce costs.

NON_PERFORMANCEDoes not identify how nurses can help coordinate care to increase patient safety with medication administration and reduce costs. BASICIdentifies how nurses can help coordinate care to increase patient safety with medication administration and/or how to reduce costs but not both. PROFICIENTExplains how nurses can help coordinate care to increase patient safety with medication administration and reduce costs. DISTINGUISHEDExplains how nurses can help coordinate care to increase patient safety with medication administration and reduce costs, providing specific examples related to a patient safety risk.

COMMENTS:

Good work explaining how nurses can help with coordinating care to increase patient safety and reduce costs. For a higher performance score, you could have provided examples of medication errors, such as how to prevent or reduce patient harm, such as participation in decision making processes for the organization.  Now that you’ve learned more about coordinating care, how do you see this as impacting your practice?

CRITERIA 4

Explain the nurse's role in coordinating care to enhance quality and reduce costs.COMPETENCY

Explain the nurse's role in coordinating care to enhance quality and reduce costs.

NON_PERFORMANCEDoes not identify stakeholders with whom nurses would need to coordinate to drive quality and safety enhancements with medication administration. BASICIdentifies stakeholders, but their relevance to collaboration with nurses or their ability to drive quality and safety enhancements with medication administration is unclear. PROFICIENTIdentifies stakeholders with whom nurses would need to coordinate to drive quality and safety enhancements with medication administration. DISTINGUISHEDIdentifies stakeholders with whom nurses would need to coordinate to drive quality and safety enhancements with medication administration, noting the relevance and potential importance of the stakeholders.

COMMENTS:

I appreciated your identification of the stakeholders that nurses need to communicate and collaboration with to implement quality and safety enhancements to reduce med errors. To achieve a higher performance score, you could have provided a more detail explanation of the relevance and importance of involving the stakeholder of the organization and their role.
