Week 3 Questionnaire Template

For the Week 3 Application Assignment, you will interview an individual in order to gain insights into how his or her religious affiliation shaped his or her experiences as a child and adolescent. Use this template as a guide to determine the questions you will ask during the interview. You may add or delete questions as you see fit, but be sure to gather enough information to complete the assignment. You will use your notes from the interview to create a 7- to 10-slide PowerPoint presentation profiling this individual’s experiences and sharing insights gained.

1. Did you grow up within a religious tradition? If not, why do you think this was the case? If so, how would you describe the religious environment in which you were raised? For example, as a child and adolescent, how often, and where, did you worship? What, if any, types of religious practices did your family participate in and/or encourage in you?

2. As a child and adolescent, what did religion mean to you? In what ways, if any, did it factor into your self-concept and identity?

3. In what ways, if any, did religion affect your day-to-day life as a child and adolescent, e.g. your experiences in school, your interactions with friends, your afterschool activities, etc.?

4. In what ways, if any, did your religious views change as you grew up, from childhood to adolescence, and from adolescence to young adulthood?

5. Do you feel that religion played a positive role in your development throughout childhood and adolescence (e.g. morally, socially, cognitively, emotionally, etc.)? How so?

6. Did you face any challenges or difficulties because of religious affiliation, or lack thereof, during childhood and adolescence? Explain.

7. What religious views were you exposed to as a child and adolescent? If you were raised within a particular faith, were most of your friends and members of your community of the same religion? Would you characterize the environment in which you grew up as religiously diverse? What effects did this diversity, or lack thereof, have on your development?

8. Do you subscribe to a particular religion today? If so, do you consider yourself religiously conservative or religiously liberal? Is this similar or different than your views as a child and adolescent? Why do you think this is?
