RE: Florence Moynihan

This is a followup note for your records on Mrs Moynihan.

As you recall she had been hospitalized on September 13 2014 at Chambers Hospital with severe right groin pain of 4-5 hours duration. She was a gravida 4 para four who, during her pregnancies, had had large painful veins in the right leg. She gave no history of previous superficial or deep phlebitis.

Examination on admission to the hospital showed marked tenderness over the right femoril vein and the long safenous vein from the groin to approximately midthigh. There was no redness or edema; homan sign was negative, and there were no obvious significant varicosities and no calf-tenderness.

Because of the severity of her symptoms, she was, nevertheless, treated as a superficial phlebitis involving the long saphenous vein extending to the saphenofemoral junction. I felt it best that she be placed on bed rest and treated with heparin. This was subsequently carried out during the next ten days. Since that time and following a switch to coumadin, she has been monitored weekly with prothrombin time and has been seen in the office.

The plan would be to keep her on anticoagulants for two-three months and then gradually taper off.
