Administrators should defined the extent of the problem and the relationship between disruptive behaviors and both patient safety and nurse recruitment/retention, we turned our attention to what we could do to make things better and to foster a safe, healthy working environment. Improving efforts such as (i) raising awareness, (ii) gaining cultural and organizational commitment, (iii) developing appropriate policies and procedures, (iv) fostering and supporting project and clinical champions, and (v) providing the necessary education to try to prevent these types of events from occurring. Staff are typically reluctant to reporting as fear of being reprimanded. The term whistleblower comes to mind along with the negative connotations. A whistleblower is a person who reports corruption and criminal and unethical business practices in the workplace to appropriate government authorities. A whistleblower provides information he or she reasonably believes evidence violation of any law, rule, or regulation; gross mismanagement; a gross waste of funds; an abuse of authority; a substantial and specific danger to public health; and/or a substantial and specific danger to public safety” (Pozgar & Santucci, 2016, p. 493). Since a physician is viewed as someone of power, often times they can misuse or abuse their authority.

With the nature of being a physician, there comes a certain level of authority that is sometimes abused. Some physicians take the power and authority that they are given and put them into actions that can cause serious damages to a hospital, their practice, their patients, and the other staff members or employees that work around and with them. When situations like the ones above arise, it is important for the medical staff, patient, or hospital to report the actions and particular situation immediately. By reporting an action this like immediately, the person or persons involved in the situation bring something to light about a particular physician but they are also protecting themselves from being treated improperly or something that could arise later and turn into a lawsuit. In reporting these actions immediately, it is important for the higher ups; in most causes would be the hospital to handle the situation immediately. It is much easier to correct a problem after it has been made verses having to fix it much later down the road and now you are having to fix more than just one.
I think that many medical staff feel as if they cannot report issues like this because of the authority that physicians are given. I think their first thought would be is this going to cost me my job, what if not one listens to me and thinks I am making it up, will this make me look like someone who just tells on people, and different things of that nature. I think that because physicians are given the authority that they have, many think that they are untouchable and I think that because of that attitude, many medical staff think that it is not even worth telling because they fear that nothing might not be done about that situation.

Disruptive behavior from a physician can include a variety of inappropriate behavior patterns that include both obvious and passive actions ranging from verbal outbursts and physical threats to refusing to perform tasks and displaying an uncooperative attitude. When a physician exhibits abusive behavior, it is intended to discredit, belittle, berate and even threaten an individual. They also use this to intimidate and undermine confidence which is one reason staff may hesitate to report the behavior. Another reason is if it is coming from a high-volume physician, the hospital may give them more favoritism because of their profitability by going easier on them, overlooking incidents, and not monitoring their behavior.
The Joint Commission on Accreditation of Healthcare Organizations (JCAHO) has implemented a zero-tolerance policy for threatening, intimidating, and disruptive behavior by requiring hospitals to have tougher policies or risk losing their accreditation (Maruca, 2011). These standards include implementing a code of conduct defining acceptable and disruptive behaviors, training hospital staff, and executing a plan for managing and holding them accountable. Hostile environments are also recognized by patients and their families. When a hospital tolerates this type of behavior, they are putting patients at risk of poor care or possibly injury.
