Purpose

The purpose of this assignment is to conduct a systematic assessment of health and illness parameters associated with a chronic, global health

issue. In completing this assignment, you will create a product for dissemination geared toward a population health setting. The ABCD

framework will be utilized to complete this activity.

Topic: Choose one of the following global health issues from the list below. Then, identify a vulnerable population that is impacted by this

illness. (For example, is a certain population more prone to this disease? Are certain environmental, geographic, cultural or other factors noted

to make a certain population more susceptible to developing and/or having complications from this particular illness?)

Appraise: Conduct an assessment of the health and illness parameters of the chosen disease within your identified population. (Consider

Community Status Indicators, CDC chronic disease data, National Public Health Service strategic plans, World Health Organization data,

Healthy People [social determinants of health] for this part of the project.)

Bounce off: Next, consider how a nurse leader might approach this issue? Search the literature for evidence to support an appropriate and

feasible intervention to address this problem in the community. Consider issues such as budget, cost effectiveness, practice initiatives, and

program impact. Formulate the following and share with your classmates in the week 4 discussion forum a) One specific intervention

addressing the population health issue, b) goal of the intervention, and c) budget and cost effectiveness measures d) methods for evaluation of

intervention. This initial post is due by Saturday at 11:59 pm PST of week 4. Gather insight from your peers through engaging in critical

appraisal of one another’s posts. (This component will be graded separately) (Initial post due by Saturday at midnight of week 4 and final peer

responses due by Tuesday at midnight along with part “D”.)

Compile: With consideration to the feedback received in the discussion forum and evidence found through your independent efforts, compile

themes to synthesize the evidence. In voice-over PPT format, discuss the problem and the plan for improvement. Address all components

explored in the previous steps of this assignment. Specifically address how this model would reflect best practices in population health as well

as the specific needs of the vulnerable population being explored. The product for dissemination should be appropriate for presentation to an

interdisciplinary team within a population health setting.

Disseminate: Share your overall findings with the other students in the course by posting your product of dissemination (as an attachment or a

link) in the discussion area of week 4 by Tuesday at 11:59 pm PST. You should review your peer’s submissions and provide feedback to at

least two peers. Because your final product is also due on Tuesday, you are not required to specifically respond to final posts. You are required

to respond to at least two peer’s discussion posts during the week. You should also submit your final assignment submission to the drop-box in

week 4 for grading by Tuesday at 11:59 pm PST.
