 As an experienced cardiac nurse of 7 years, it has been proven that we see repeat patients regularly on our unit. This is typically due to a lack of education, monetary means for attaining their medications, difficulty with transportation to follow-up appointments, or non-compliance with their discharge instructions. One such situation that stood out was when a patient was prescribed Warfarin (Coumadin) upon discharge. It is an oral anticoagulant that is usually prescribed to patients with deep vein thrombosis, pulmonary emboli, and atrial fibrillation (Nature video, 2016)[video]. The patient was not certain how he was going to purchase the medication or get to his follow-up appointments. The patient also voiced that there was a lack of understanding, on his behalf, of what the medication was being prescribed for. The charge nurse and I agreed this was an unsafe discharge. We immediately notified the provider and reached out to the case manager to help educate the patient to resolve these problems and make this a safe discharge.   

As a provider, I would have applied my knowledge of how pharmacokinetics react in the body and pharmacodynamics is the process of what the drug does to the body to explain to the patient the need for this medication (Speed pharmacology, 2015)[video]. Understanding that pharmacokinetics to drug therapy means to help maximize beneficial effects and minimize harm. With these factors and my knowledge, I would have verified that the patient understood why the medication was prescribed, its usage, side effects, and affordability. I would have the case manager involved assist the patient with attaining means of transportation to appointments for frequent blood work, therefore ensuring the medication was working properly. Through the follow-up blood work, the results would dictate that the beneficial effects of the medications were being achieved without causing harm through doses that were too high demonstrating a proper medication balance. As a provider, it is our duty to promote safe practice when prescribing and managing medications, resulting in reduced return hospital visits, and preventing unnecessary harm to the patient (Rosenthal, L,D., & Burchum, J.R. 2021).
